Safety After Discharge

 

Not only should health care organizations be concerned about the safety of patients while they are admitted to the hospital, we also must be concerned about their safety after discharge. The Agency for Healthcare Research and Quality (AHRQ) has released a Patient Safety Primer discussing hazards that patients face. 20% of patients suffer adverse events after discharge, the most common of these are adverse drug events. Care transitions are also a concern, as ineffective communication between inpatient and outpatient providers often occur. A multi-modal approach to improve this is suggested, including emphasis on medication reconciliation, structured communication pathways and patient education. As financial penalties are increasingly placed on hospital readmissions, this topic becomes even more important for health care system viability.
