The American Society for Healthcare Risk Management (ASHRM) has published a tip sheet to promote patient safety. A sampling of the information:
- The majority of harmful adverse drug events are associated with the use of anticoagulants, narcotics, sedatives and insulin.
- To prevent catheter associated urinary tract infections: wash your hands, never tug or pull on the tubing and be careful not to let it kink or twist.
- Central line associated blood stream infections are associated with over 25,000 possibly preventable deaths every year.
- Case management, care transition management and discharge phone calls are all effective strategies in decreasing preventable readmissions.
