2014-15 School Forms Available On-Line

All Enrolled Students for 2014-15

 

Please complete the following two important forms by August 1

 

1. Click here to complete our NEW On-Line Student Emergency Form 

In an effort to make sure that we have the most up-to date information for everyone and with our transition to a new electronic system, everyone must complete a blank form this year. Please fill out one form for each enrolled student. We apologize ahead of time to families with multiple children in the school! 

PLEASE COMPLETE THE FORM BY AUGUST 1

 

2. Click here to download and print the Signed Authorization Form for 2014-15 which includes:

-Emergency Care Authorization

-Permission for GMS to Administer Benadryl

-Photo Release Authorization

-PLEASE PRINT AND RETURN THIS FORM TO GMS BY AUGUST 1

-The document can be scanned and emailed to Lindsay Jeffrey at the front desk

 

** NC Kindergarten Health Assessment Form - All returning Primary students who have turned 5 this past year or will have their 5th birthday by Aug 31, must have their physician complete the 

NC Kindergarten Health Assessment Form. 

 

*** Rising 6th Graders - please provide GMS with documentation of any and all updated immunizations, including the required dose of the Tdap vaccine. 

 

 

New Students in 2014-15 - Additional Forms

 

New Students also have two additional forms to return to GMS. The documents can be scanned and emailed to Lindsay Jeffrey at the front desk.   

 

1. Discipline Policy Acknowledgement Form - PLEASE COMPLETE BY AUGUST 1

- Parents of all New GMS students must sign and return a copy of our Discipline Policy

 

2. Health Forms - Due no later than September 20, 2014

- All NEW GMS students need to submit a Children's Health and Medical Report with immunization history by Sept 20

- New Kindergarten age students submit the NC Kindergarten Health Assessment Form instead. 

 

 

CASA Registration Form for 2014-15

 

Some families have already registered for before and after school care for the coming school year. Please contact the front office to confirm your registration.  Or complete the following CASA Registration Form and return to GMS as soon as possible. 

 

 

 

School Supply Lists

 

Toddler, Primary & Lower Elementary students do not need to purchase school supplies for class.

 

Upper El Supply List was emailed to all Upper El Families this week

 

Middle School Supply List will be available by August 1.

TRAILBLAZERS EXTREME SUMMER CAMP  
 JULY 28- AUG 1
 
Camp leaders are John Archambault (GMS Upper El teacher) and Morgan Kratovil (former GMS Middle School teacher)
 
TRAILBLAZERS EXTREME  
(Rising Upper El & Middle School students)
This is a very popular camp for those who love being outdoors and exploring nature. The focus of this camp is to have fun hiking through our state parks and national forests. Hikes are fast paced and cover several miles per day. Some days may be quite strenuous. In addition to the hikes, hikers will swim, practice good hiking etiquette, and learn basic wilderness survival skills. The experience will promote an appreciation of our state's resources and of nature itself. Campers will meet in the office each morning and return by 5:30pm daily.

    Faucette Farms CSA Pick Up Continues This Summer
     
    All of our veggie-loving families will be happy to know that Faucette Farms will once again offer CSA pick ups all through the summer!  If you currently participate in the program, mark your calendars and plan to stop by and pick up your bag of fresh, organic produce each Wednesday between 2:30pm and 3:30pm.  Elmer or Mike with Faucette Farms will be there to deliver your bag of farm fresh, organic produce-- just look for their familiar truck parked on the hill (grassy lot to the left of the school).  
     
    If you are interested in joining the CSA program, please email Polly Evans at Faucette Farms at [email protected], or call her at (336) 669-5083.  You can also visit www.myfreshproduce.com and fill out an online application, then call/email Polly to let her know you have joined.  You can choose from a small, regular, large, or extra large share of produce each week, with a commitment of 12 weeks at a time.  You also get to select the produce that you would like each week by filling out an online order form (www.myfreshproduce.com) by the Monday before the Wednesday pick up times-- a definite perk of the program.  If you have any questions about the program, you may also contact GMS parent, Ellen Lloyd, for more information-- [email protected]
