Arizona Foundation's Online Enrollment Verification Tool 

Arizona Foundation offers providers on-line patient enrollment verification.

This information DOES NOT guarantee coverage or payment, but rather indicates that the specified patient has access to our network for the date of service, according to enrollment records.

To verify benefits and eligibility, you MUST contact the Administrator listed on the enrollment screen or from the patient's health plan member ID card.

It's Here! New Web site Announcement
Arizona Foundation for Medical Care (Arizona Foundation) is pleased to announce the release of our new Web site.

With a completely new user interface, the site offers innovative features and unique functionality. From the redesigned logo to the online contact us form, Arizona Foundation's Web site provides visitors with accurate, current and well organized information for making your relationship with Arizona Foundation even more rewarding.    
                    
When entering the new site, look for the Provider link at the top of the page, which allows you to find all information relative to you; it was designed with you in mind! Be sure to check back often as we are working on expanding this area.

Site of Service Differential

Effective January 1, 2013, Arizona Foundation will implement a Site of Service Differential methodology for physician services rendered within a facility setting.  

 

Facility settings are defined by the place of service codes listed below:

 

 

 

Arizona Foundation will render a higher reimbursement to the physician when certain procedures are performed in the office rather than a facility setting to compensate the physician for the overhead costs they incur. When these services are provided in any of the places of services listed above, it is the facility that incurs these costs and those costs are already reflected in the fee paid to the facility.

Arizona Foundation's Renewal Season is here! Starting September 1, 2012 and continuing throughout the end of the year, providers must renew their network participation for the 2013 calendar year.

  

Arizona Foundation provider members have two time-saving options for completing their renewal:

  1. Renew online at www.azfmc.com using your license number and Password PIN supplied in the letter mailed or e-mailed to you at the beginning of September. We strongly encourage all providers to take advantage of this time saving feature.    
  2. Download your renewal form online using your license number and Password PIN. Complete the renewal form and mail it along with your membership dues (if applicable) to Arizona Foundation, Attention: Provider Relations, 326 E. Coronado Rd., Phoenix, AZ 85004.

Patient Referrals

Arizona Foundation is a Preferred Provider Organization. As such, plan members who go to in-network providers receive higher "in-network" benefit levels than they would by seeking medical services from out-of-network providers.

 

We ask that you refer plan members accessing the Arizona Foundation network to in-network providers to minimize their out-of-pocket expenses, especially for lab services, pathology groups and DME providers.

 

You can find other in-network providers by using the Arizona Foundation Provider Search Tool. If it is necessary to refer a member out-of-network, please remember to inform your patients of that referral before the service is provided.

Arizona Foundation's Provider Change Form, located on the new Commonly Used Forms page on the Arizona Foundation Web site is a way for providers to keep us informed about current practice information such as a change of address, Tax ID#, or specialty change.
