Hungry and Can't Lose Weight?

Trouble starts when your brain ignores your fat

It is amazing that we used to think that weight loss was a simple matter of eating less and exercising more. Eat fewer calories than you burn and you'll lose weight. If this were true, maybe an accountant is a better choice than your doctor in seeking help with weight loss. I do believe that the calorie consumed-calorie burned equation is of some validity. However, in between eating food and burning calories is a big, mysterious factor called metabolism.

Metabolism and weight loss is a very complex subject. Some of the important factors in weight loss are hormones. I think that just about everyone has heard that an underactive thyroid can interfere with weight loss or cause weight gain. The fact is that there are a multitude of hormones and signaling molecules in the body that influence fat formation and fat burning.

One of the important hormones related to fat loss is leptin. Years ago, leptin was discovered in animals and was determined to have two very important effects in regard to weight loss: it shut off appetite and increased the burning of body fat. Leptin, it would seem, is a dieter's best friend. Drug companies raced to develop a leptin-like drug. These efforts came to a screeching halt in the early stages of human research. It was hypothesized that, given the effects of leptin on appetite and metabolism, obese people must have inadequate leptin levels. When obese people had their leptin levels tested they were found to have elevated not depleted levels. That didn't seem to make sense.

Actually, obese individuals do have a leptin problem - leptin resistance. It is one thing to have a certain level of hormone in the blood but that amount does not guarantee a reliable corresponding level of hormone activity. For example, if there is a shortage of cellular receptors for the hormone, reduced activity results. In leptin resistance the leptin does not work well. The high blood level is deceiving. As far as the brain is concerned, there is a leptin deficiency.

How's that leptin working for you?

Leptin is produced by fat cells. In a healthy state, when your fat cells get a little engorged they secrete more leptin into circulation. The
brain senses the increased leptin level and responds by reducing appetite and increasing the metabolic rate (number of calories burned). With leptin resistance, you lose these beneficial effects on appetite and fat loss. So, what causes leptin resistance to develop?

One factor that contributes to leptin resistance is frequent consumption of high glycemic index foods. These are basically found in white sugar and flour products (cookies, cakes, candy, bread, pasta, cereals, muffins, biscuits, bagels, soda, etc.). Sugar causes leptin to spike. When this happens over and over again, the brain begins to tune-out the leptin signal.
Leptin Resistance

Another important contributor to leptin resistance is chronic inflammation. There are many factors that induce a chronic inflammatory state in the body. A few of these are the Standard American Diet (SAD), toxicity, dysbiosis (imbalance in intestinal organisms), stress, and inadequate sleep. High levels of the stress hormone cortisol impair the functioning of leptin. Cortisol levels are best measured from saliva specimens collected at four times over the course of a typical day. For more on this subject see my article on cortisol and DHEA.

There are a number of strategies for restoring the normal activity of leptin in the body. Increasing intake of protein is often appropriate, as is eating more monounsaturated fats (nuts, avocado, olives). Fish oil can help reverse leptin resistance.

Exercise not only burns calories but helps to improve leptin sensitivity. Reducing the amount of well-cooked foods may also help. There are also a number of nutritional supplements that improve leptin resistance. Fat loss is both a goal and a treatment; fat loss itself can help reverse the leptin resistance.
