New Resources

mygene2.org - mygene2 is a website developed by scientists and clinicians at the University of Washington to connect families, clinicians and researchers through sharing their stories of rare genetic conditions.
 
Presentation materials for "Coordinating Care For Kids - School Nurses Linking With The Medical Home" webinar can be found by clicking here. This webinar was presented in May 2016, and provides practical examples and resources on how school nurses, community-based clinicians and other providers can implement care coordination successfully.

From WA State Dept. of Early Learning: Practice Guide Materials For Infants & Toddlers With Sensory Disabilities - a practice guide to inform providers on services for infants and toddlers with sensory disabilities.  The guide includes information on eligibility, pathways to services, and other resources for families.  A self-evaluation tool has also been developed for agencies to help LLAs and EI agencies evaluate their readiness to support families with newly identified birth-to-three year olds with sensory disabilities.
