Be one in a Million Hearts
|
In September 2011, the Department of Health and Human Services launched the Million Hearts initiative to prevent 1 million heart attacks and strokes by the year 2017. The initiative focuses on improving the ABCS (Aspirin therapy for those at risk, Blood pressure measurement and control, Cholesterol measurement and management, and Smoking cessation).
There is a need to target and provide assistance to home health providers and their patients to support the initiative and prevent heart attacks and strokes. Consequently, the HHQI National Campaign will be working with CMS to include the home health community in the Million Hearts work.
