New Psoriasis Referral Form 

In response to the needs of GP's in practice, the Irish Skin Foundation - in conjunction with a leading Consultant Dermatologist - has developed a practical referral form to support timely and appropriate referral of psoriasis patients to secondary care Dermatology Services. 

 

The form provides the information necessary for the consultant to readily assess the severity of the patient's psoriasis. It is automatically populated with patient details and can easily be saved to the patient's record.

 

Features include :

 

* DLQI Quality of Life Questionnaire - provides an accurate account of the effect that psoriasis has on the patient's quality of life.

 

* Body Surface Area Diagram - a visual representation of the extent of the patient's psoriasis.
