How "NOT" To Be Denied Coverage
The "Pumpkin" Rule

This may be the most important article I have ever written.
In fact, if you have come to one or more of my "Raising UP Your Parents" seminars, you know that I always:
1) emphasize that Medicare is not long term care and does not act like a long term care policy and
2) plead with everyone to make sure that if you are discharged from the hospital your physician lists your stay as having been "admitted" as opposed to "under observation."
I do not want any of you to be denied coverage for this or billed separately for every procedure and drug, which will cause your co-pays to mount until you owe thousands of dollars under Part B.
I have now drilled it into everyone's heads that post-hospital nursing home care is only covered by Medicare upon discharge from a 3-day qualifying inpatient hospital stay. Many people really are in the dark about this so consider yourself lucky.
In fact, last year there were over 600,000 citizens who were admitted to hospitals as "outpatients" on "observation status," many receiving the same care as inpatients.
To combat this, several physicians have suggested that if you have to go to the hospital, try to get there before midnight as if you cross two midnights, you're an inpatient for purposes of Medicare Part A (surgical procedures, diagnostic tests and other treatment).
A new Medicare regulation - the so-called "pumpkin rule" requires doctors to admit people they anticipate staying for longer than two "midnights," but to list those expected to stay for less time as observation patients.

Link between Alzheimer's and pesticide DDT found by Rutgers scientists
Researchers at Rutgers University have become the first scientists to link the banned pesticide DDT to Alzheimer's disease, according to a study published yesterday in JAMA Neurology.
The study examined levels of the chemical DDE, a byproduct of the pesticide DDT, in the blood of healthy patients compared with patients with late-onset Alzheimer's, the most common form of the illness.
The DDE levels were almost four times higher in the Alzheimer's group. However, not every Alzheimer's patient studied exhibited those high readings.
"Alzheimer's is a complex disease and there isn't one single factor driving it," cautioned study author Jason Richardson, of the Department of Environmental and Occupational Medicine at Robert Wood Johnson Medical School.
Although DDT has been banned in the United States for more than four decades, it still shows up in 75 percent to 80 percent of blood samples monitored by the U.S. Centers for Disease Control and Prevention.
In recent years, researchers have focused on locating specific genetic defects responsible for this type of dementia, Richardson said. The Rutgers study offers further evidence that genes alone probably aren't the whole explanation; pesticide exposure may play a role.
He cited a colleague's saying: "Genetics loads the gun. Environment pulls the trigger."

According to Kaiser Health News, the amount of people admitted on "observation status" increased 69% between 2006 and 2011, prompting the Center for Medicare Advocacy to file a class action lawsuit.
In addition, a bill, the "Improving Access to Medicare Coverage Act of 2013" is pending in the House (H.R. 1179) and Senate (S. 569). This bill would count all time in the hospital for purposes of determining Medicare coverage of skilled nursing care, regardless of such classification.
