Palliative Pupus on the 11th (January only)

This is a reminder: The first Palliative Pupus of 2017 is today Wednesday January 11.

Palliative Pupus: Come participate in an interactive interdisciplinary story from the cutting edge of hospice and palliative care and mingle with colleagues.

For the rest of the year we meet as usual bi-monthly on the 3rd Wed. of the month.

Our speaker will be Gary Powell from the Caregiver Foundation talking about the services they offer, especially around decision-making when someone no longer has the capacity to decide for themselves.

Tue. Feb 7, Webinar: The Importance of Advance Care Planning

Join us for this webinar featuring our very own Jeannette Koijane, Executive Director of Kōkua Mau. Jeannette will give an overview of Advance Care Planning, specifically focusing on Advance Directives and POLST.

Cost: $39 per registrant for HAH members; $59 for non-members.

Those attending the session will be able to:
  1. State at least 2 differences between an Advance Health Care Directive and a POLST.
  2. Describe the type of patient for whom the POLST form is designed.
  3. Understand efforts underway in our community to promote advance care planning.
  4. Identify available education, materials and resources in our community to help professionals and lay people access advance care planning.

Target Audience: Health professionals who work with people who may be facing serious illness including Social Workers, Directors of Nursing, Clinical Managers, Resident Care Managers, RNs, LPNs, Certified Nurse Aides, Caregivers, Administrators and Executive Directors
