2015 Membership Drive

Membership and donations are an essential part of keeping Kōkua Mau fiscally sustainable and we are sending this reminder in the hope you renew or join us soon.

Individual Membership starts at $40. We especially encourage organizational membership by community leaders that support our mission. Please contact Jeannette for details.
How do I join?
 
Postal Mail:
Please download our membership form and put it with a check in the good old postal mail.

Online:
Chose your membership level and click the Donate Now button on the right hand side of the webpage. Please put 'membership' in the 'Designation (Optional)' field, click 'add to giving cart' and follow the subsequent steps. We know, the free version of network for good does not look exciting or has any Kokua Mau logo, but it is free. Thank you!

Register now: Sat. 3/28 POLST Training

Kōkua Mau, invites you to a 4 hour training for all professionals.

Sat. March 28, 2015
12:30-4:30pm.
(Registration and lunch beginning at 11:30am)

The purpose of this workshop is to improve the effectiveness of the POLST discussions, especially for providers who can complete and sign the POLST form - Advance Practice Registered Nurses (APRNs) and Physicians.

This workshop:
  • Reviews the POLST orders
  • Trains you in initiating POLST and Advance Care Planning (ACP) conversations
  • Utilizes tools such as the ACP decision video and role-playing
Our faculty are experts from our Hawaii community, all with direct experience of discussing and completing POLST (Provider Orders for Life-Sustaining Treatment).

Updated Hawaii Advance Directive

A big Thank You! to the active team that made the Updated Hawaii Advance Directive possible.

After 10 years we have updated and revised the Advance Directive to make it more user friendly and accommodate the wishes of patients and their loved ones in more detail. Please use these free Advance Directives with clients, family and friends as a way to document your conversations about wishes for end-of-life care and appoint your health care power of attorney. 

Now a 3-page document, you can find the PDF file of the Advance Directive on our dedicated web page. All "old" two page Advance Directives are still valid, although we recommend you use only the new form and may consider updating an "older" form with the more detailed options available now.

Next steps will be translating this new Advance Directive into 10 languages so we can help to expand access to those who are more comfortable in another language.  If you have foreign language skills and would like to help, please let Jeannette know.
