Family Satisfaction Survey!

Dear Military Family Member,

As part of the contract under which the National Guard Child and Youth Program operates, we have been asked to survey Families who have utilized Child and Youth Program resources and services from the period of 1 June 2014 to present. The purpose of this survey is to gather valuable information related to current strengths of the program as well as areas where continued development could be targeted for improvement. Ultimately, our goal is to ensure the state/territory Child and Youth Program is adequately meeting the needs of our military-connected population.

We invite you to take a few minutes (10-15) to complete the Family Member Satisfaction Survey online. You may do this by clicking on the following link: https://www.surveymonkey.com/r/2015CYSSFamilySurvey Information gathered from this survey is completely anonymous and will only be used to improve services provided by the Child and Youth Program at the state/territory and national levels.

If you are unable to complete the survey online, a PDF version can be found here. Please mail or email the PDF version of the survey to ATTN: Ms. Erin Berry, Eastern Region Assistant Program Manager, 434 Chase Road, Columbus, OH 43214 or erin.berry@cog-ps.com

This is a community-wide plan to improve children's health! It includes looking at what you should eat and what you should do to stay healthy!

* Walk, skate, or cycle more, and drive less.
* Do stretches, exercises, or pedal a stationary bike while watching television.

* Most important - have fun while being active!

Another great way to stay active is to enjoy parks in your area and activities from your cities Parks & Recreation Programs. How to Create Shadow Photos
Some dads appreciate pictures of their family the most - here is a unique take on family photos!

Hero Dad Sign

Supplies:
- Piece of wood, can be painted or stained
- Paint for hand prints
- Piece of paper printed or hand crafted with your favorite saying, such as "We have a HERO and we call him DADDY"

Instructions:
Have each child using the same hand place a hand print in paint on the board from oldest child to youngest child.  Make sure to leave enough space for your saying or quote to the left of the hand prints. Let dry completely, then attach the saying.  This can be attached with glue or nailed to the wood.
