In the News: Payment Reform - Behavioral Health Services and Bundled Payments

BOSTON, MA  One of the cornerstones of the Patient Centered Medical (Health) Home is payment transformation. Moving from fee for service, paying for visits, to paying for health has been part of the rhetoric of the PCMH from the beginning. What the original designers of the PCMH did not realize is that this transformation in payment approach can launch behavioral health into a central role in primary care. Because the delivery regulations and payment structures that support mental health and substance abuse services were designed to be separate from medical payment and delivery, most programs that tried to integrate the two have faced complex "work-arounds" to try to make programs work financially. As soon as primary care settings are paid for health, what are generically called "bundled payments," this all changes.         

Case Management and Behavioral Health is a Key Component of an Effective Intervention for High Utilizing Patients

CHAPEL HILL, NC - Researchers in the department of Family Medicine from The University of North Carolina in Asheville and Chapel Hill have developed a promising intervention to reduce Emergency Dept use among high utilizers . The intervention, which was created for high risk, high utilizing low income patients combines case management and twice weekly drop-in medical group appointments which were staffed by a family physician, a behavioral health specialist and a nurse case manager.

PROVIDENCE, RI -  CurrentCare, Rhode Island's statewide Health Information Exchange (HIE), has become the first in the nation to receive continuous updates in real-time with behavioral health patient information and automatically integrate with medical information, providing a comprehensive patient health profile.

ENTERPRISE, OR -  Wallowa County is a remote, frontier county nestled in the northeast corner of Oregon.  With only 7,000 total inhabitants and isolated from the rest of the State by geographic barriers, people in Wallowa County often have to be creative and innovative in order to make things work.  Recently the State of Oregon embarked on a system transformation effort designed to change the way Medicaid services are provided that is focused on integrated care.

CAMBRIDGE, MA -- "Primary care behavioral health services, which embrace mental health, substance use, and health behavior change, are the foundation of the nation's mental health care system.  More mental health care is rendered in the primary care setting than anywhere else, including mental health care settings.  Here we find enormous numbers of common mental and substance use disorders that are neither simple nor minor. 

Since most patients are unwilling to accept a mental health referral outside the primary care setting, these problems are addressed in primary care, or not at all.  Yet, when the primary care practitioner is the only behavioral health provider, treatments are marginally effective and often frustrating to patients and providers."
