Dear Northfield Township Unincorporated Residents,

As a result of last week's heavy rainfall and subsequent flooding in the unincorporated areas of Northfield Township, Cook County Board President Preckwinkle issued disaster declaration for Cook County.  Governor Quinn has subsequently issued a disaster declaration for the State of Illinois encompassing more than 40 counties across the state.

The President has not yet issued a federal disaster declaration for our area; and the Governor's request is first subject to review by the Federal Emergency Management Agency (FEMA). Before the federal government conducts its assessment to determine whether a federal disaster declaration may be issued, local jurisdictions are required to capture preliminary damage assessment information and provide this to the County and State emergency management agencies.

Northfield Township is coordinating with the Cook County Department of Homeland Security and Emergency Management (CCDHSEM) and the Illinois Emergency Management Agency (IEMA) to collect initial assessments of the impact of last week's storm from residents in the unincorporated areas of Cook County.

Northfield Township is requesting the assistance of residents in the unincorporated areas only; to help us develop the most accurate and up to date information for CCDHSEM and IEMA by completing the following survey

http://www.surveymonkey.com/s/CW3MRHZ

PLEASE NOTE THIS SURVEY IS ONLY TO BE COMPLETED BY RESIDENTS OF THE UNINCORPORATED AREAS OF NORTHFIELD TOWNSHIP. IF YOU RESIDE IN ONE OF THE INCORPOATED VILLAGES WITHIN THE TOWNSHIP PLEASE CONTACT YOUR LOCAL VILLAGE FOR FURTHER INSTRUCTIONS

The survey is quick and easy to complete, and will assist the county and state in their efforts to secure a Presidential Disaster Declaration, which could create the opportunity and potential eligibility for financial assistance  under FEMA's Individual Assistance program.

Please note that this survey is only being used to develop preliminary damage assessment information, and completing this survey is not an application for any form of financial assistance or reimbursement at this point in time.

If you have questions or concerns please contact
