Care Navigation Services

The Alzheimer's Care Resource Center offers Care Navigation Services. These services are beneficial in assisting families who are most often, employed, have families and/ or may be long distances from their loved one.  A Geriatric Care Navigator is a professional social worker, counselor or nurse who specializes in geriatrics by assisting seniors and their families to formulate, implement and monitor care.

 

Care Navigation services allow family members the ability to have a caring, educated and experienced local contact who can provide ongoing assessments of their loved one, monitor and direct care provided at home, provide financial, legal and/or medical reviews, provide referral services to qualified specialists and community resources as well as counseling, support, and caregiver coaching services.

 

The Geriatric Care Navigator will begin by conducting an in-home comprehensive assessment which identifies the problems, assesses the level of care needed and finds the solutions by developing of a personalized care plan for your loved one. The care plan can include:

  • Crisis Prevention, Intervention and Stabilization
  • Medication management
  • Acting as liaison between physicians, families, client and other health care professionals
  • Ongoing assessments and updates for family
  • Relocation Assistance
  • Caregiver Coaching
  • Education and Referral services
  • Coordination of medical appointments and medical information
  • Review of financial, legal or medical issues
  • Provide advocacy regarding medical, financial or legal needs
  • Monetary management
  • Support and counseling services
  • Assistance with Medicaid qualification and application process
  • Oversee and direct care provided at home
  • Fall prevention and Home Safety Evaluations
  • Emotional Support
  • Nutrition and Dietary management
  • Health and Wellness Services
  • Community Based Services and Entitlement Evaluations
  • Home Management Services

 

These services are customized for each client and family's individual needs and situation.

 
Caregiving is a full-time job. It is important to remember, you do not have to do this alone! By incorporating a Geriatric Care Navigator families benefit from peace of mind that someone is local, will help reduce stress, depression and monetary expenses.

 

I am a Care Navigator for the Alzheimer's Care Resource Center. I hold a Master's in Art of Clinical Psychology. I have worked with seniors and families coping with Dementia for over 20 years. I joined the Alzheimer's Care Resource Center because of my passion to assist seniors and families. I find it very rewarding to assist seniors in maintaining their independence and supporting the family members in understanding and coping with all the health concerns for their loved one.

We've created these helpful Alzheimer's tips and points to ponder to help you manage in your role as an Alzheimer's caregiver.

 

  • Be prepared - Make sure this is really a role you can undertake. In some cases you may not have a choice, but many times you do, even if it means pushing back on other family members.
  • Be realistic - Care giving takes endless patience and stamina. Pace yourself. You are in it for the long haul.
  • Protect yourself - Don't try to do it all. Ask for help
  • Be organized - You will quickly become the center of a chaotic universe. Set up whatever system works best for you to stay organized.

Therapeutic Thursday

therapeutic thursday The number of male caregivers has often been unrecognized, but not anymore. In the last 15 years, the percentage of men caring for a loved one with dementia or Alzheimer's disease has more than doubled, from 19% in 1996 to 40% in 2009, and has continued growing rapidly! Too many times we see male caregivers who try to do it all alone but those who seek outside help with tasks are less likely to burn out and better able to provide long-term support for their loved one.

 

Male caregivers can find some solace, healing, and community by reaching out to others who share their experiences.  But understanding how men cope and seek support is essential to ensuring the male caregiver has access to the support, professional programs and information they need.

 

The Alzheimer's Care Resource Center is now offering Therapeutic Thursday videos, which are just for our male caregivers. Each month, on our YouTube channel and on our website, we are featuring a new series, called Therapeutic Thursdays™ which will provide even more in-depth, male-caregiver focused content.

 

We are so excited that these new programs and services will help further our mission of helping male caregivers to relax, renew and reach-out and help meet their own unique self-care needs.

Alzheimer's Caregiving DVD's Sold Here

The Alzheimer's Care Resource Center has been teaching Alzheimer's Disease Caregiver Education to family caregivers since February of 2003. Our Alzheimer's Caregiver DVD Series provides education, tips and techniques for caregivers of those with Alzheimer's, dementia or other memory impairment.

 

Whether you are looking for information on managing difficult behaviors or struggling with communicating with the Alzheimer's patient, you will find an educational DVD that will help and that you can watch in the comfort of your own home.

 

We have recently expanded the Alzheimer's / dementia specific educational services we offer to include topics that are important to caregivers and families suffering from Alzheimer's disease, dementia or other memory impairments.

Support Groups for Caregivers

Caring for someone that has Alzheimer's disease, dementia or other cognitive impairment is hard. Our support groups are attended by caregivers who are struggling with all kinds of different challenges and emotions.

 

Our West Palm Beach area Alzheimer's caregiver support groups are different! Our caregivers are able to come together and share in a warm, relaxed and nurturing environment. We also offer a monthly telephone support group for all Alzheimer's caregivers who are either unable to attend in person, or live out of State.

 

The Alzheimer's Resource Center's calls our family of supportive programs Caregiver Cottage. Programs are designed for family and professional caregivers of individuals with Alzheimer's disease or a related dementia.
