| A Word About The Alzheimer's Patient |
When we started the Alzheimer's Care Resource Center, we knew that our mission would solely revolve around meeting the needs of the Alzheimer's caregiver. That's not to say that we are not fierce advocates for the Alzheimer's patient. We are. But we recognize and strongly believe that there are some wonderful organizations throughout Palm Beach, Broward and Martin counties that already do an outstanding job in meeting the needs of the Alzheimer's patient. From home health agencies, nurse registries, memory disorder centers, physicians, assisted living facilities and nursing homes, to all of the community based organizations in each county, the Alzheimer's patient is continually surrounded by individuals who work very hard to assure they enjoy the best quality of life possible. We support and applaud everything that they do. But the Alzheimer's Care Resource Center is all about YOU! About your needs, your struggles and your quality of life. We know that you are trying to navigate your way through each and every day and we are here to help you chart the course. By helping YOU, we help the Alzheimer's patient in a very, very important way... By keeping you healthy, keeping you happy and keeping you empowered to be the best caregiver you can be for your loved one. With warmest regards, Elayne Forgie President/CEO |

| Geriatric Care Navigation |
Our Geriatric Care Navigation Program™ was designed as a progressive, affordable and empowering approach to traditional geriatric care management to offer a professional family for caregivers to turn.
Caregivers are often overwhelmed with making choices for their loved one. Coping with current care needs and trying to anticipate the future can create stress and very often fear of the future. The good news is caregivers never need to feel alone and can feel hopeful for that even in complicated situations, solutions exist to make a positive difference for the caregiver and the care receiver.
All of our care managers are social workers, registered nurses, counselors or geriatric care managers who provide professional expertise and experience with the dementia journey to help assess individual needs, thoughtfully craft appropriate care options, and empower caregivers and families to make choices to enhance their loved ones safety and quality of life through a Geriatric Care Navigation™ plan.

The Reef: Meeting the Unique Needs of Male Caregivers
|
Male caregivers are a vital emerging group in our mission to provide dementia specific services and education.
We've been concerned that not enough male caregivers are taking advantage of what our organization has to offer and have just launched a special suite of programs and services, called "The Reef" which is just for them.
|
| About Us |
The Alzheimer's Care Resource Center is the only non-profit organization in Palm Beach, Broward and Martin county focused strictly on meeting the needs of Alzheimer's and dementia caregivers. We are so excited that we can bring these wonderful programs and services to the community and further our mission of helping caregivers to relax, renew and reach-out to help meet their own unique self-care needs.
Sincerely, Alzheimer's Care Resource Center |
