Service Excellence Standard of the Week
Service Standard #10: Teamwork

From a resident at New Horizons regarding caregivers Douglas Machapu and Solomon Sarfo Douglas is a very kind and caring man, putting on my socks and shoes. He noticed my skin by the ankles and feet was very flakey, so he lotioned and massaged that area. This denotes extra loving care, much appreciated by me. Please thank him. Please thank Solomon also, for taking care of me and putting lotion on my feet. Solomon gives goes beyond to assist me. Please thank him. Submitted by Michele Labrecque out of the Quincy office about the Referral Center I think we are all so quick to criticize when something has gone askew...however not as quick to inform the correct parties when something is going well. So I just wanted to bring to everyone's attention how wonderful Kathy in referrals has been when requesting referral information. Kathy will always call me in a timely manner, is pleasant and well informed. Just thought I should let you know. Thank you! Submitted by Maria Dunn, Patient Care Program Manager about Erin Parks, RN in the Danvers office Today I randomly chose a chart to provide an example of a clinical summary to assist with 485 education. The first one that I looked at was one from Erin Parks! I wanted to compliment her on her assessment documentation; it is exactly what I was looking for in the SOC clinical summary! It paints a clear, concise picture of the patient and was so easy to follow. Looking at the SOC OASIS Change Form, her OASIS documentation was great too!!! It was a pleasure to view her documentation today! Thank you for accurately conveying the patient's acuity in your OASIS, clinical summary and most importantly the patient's POT! Submitted by Laura Lawrence, Administrative Assistant VNA Care Advantage /New Horizons Office I just want to thank Denise Mandeville, Administrative Assistant to Karen Chirsky, Vice President, Transitional Care & Business Development and Kelly Frew, Director of Marketing & Public Relations. Denise is always available to assist and help our office whenever asked. She always goes above and beyond to help us out even with her busy schedule. She is always professional and such a pleasure to work with; she has a great sense of humor. Denise is and continues to be a tremendous help to our office. Thank you so much Denise for all you do. Submitted by Wendy Drake, Rehab Services Manager in the Worcester office on behalf of a happy patient I received this letter from a happy patient who had Robert Dulfan as his physical therapist: "Rob Dulfan was excellent! He was very knowledgeable of my surgery and the therapy that works best for me, and how to manage my pain. He should be recognized for his work, he knows his job well. He was a big help to me, and I am doing so much better!" Great job, Rob! Submitted by Meryl Johnson, hospice liaison nurse I just received a call from a daughter of a former patient of ours. Patient S.G.'s daughter called to let me know that her father passed peacefully in Tobago. He passed on the 17th of this month with both of his daughters at his side singing to him as he died. She said that it was beautiful. She specifically called to let me know how wonderful her father was cared for while he was at her home in Boston with our hospice. She said that Augustine Abunaw was a saint, took wonderful care of her father and provided support for the family. She told me how fortunate that her father was to be under his care. She also praised the hospice aide that did wonderful personal care for her father, shaving him and making him look and feel so good. She could not remember the name of the aide. She asked for our address so that she could send a personal note to thank everyone. Our team works so hard, so it is great when someone takes the time to let them know that they make a difference. Thank you!

Awards, Appointments and Recognition
Robin Wilson Congratulations to Robin Wilson, MSN, RN, CHPN, Palliative Care Services Manager for VNA Hospice & Palliative Care, who was elected to serve on the Board of Directors of the Hospice and Palliative Care Federation of Massachusetts at its annual meeting on May 11th. Erica Friend Congratulations to Erica Friend, RN in the Worcester office, who diligently pursued her BSN amongst many other competing demands for her time, all while working full time. Erica accepted her diploma at graduation on May 21.

Special Announcement
It is with great excitement and pleasure that I announce, effective July 1, 2016, all Social Workers throughout the agency, Community Resource Specialists and Behavioral Health Clinicians, will become one team under the Behavioral Health umbrella.
Social Workers and Community Resource Specialists provide valuable resources, support and a wide variety of services for patients and families. The behavioral health clinicians provide opportunities for patients to add to or adjust their current lifestyle leading to a higher quality of life. Thus, when viewing MSW, CRS and BH as "one team", the ability to collaborate, to build a stronger and more cohesive foundation and to fortify resources across VNA Care only strengthens our mission to help our patients, families and clinicians through many of life's challenges . The goal of this "one team" approach is to encourage MSW'S, CRS and BH clinicians to work together to best meet the needs of the patient. The wealth of resources and knowledge amongst this group is exceptional. By combining these experts onto one team, the referring clinician will have the comfort and reassurance that making one call to any member of this team will spark an immediate team focus to assist the patient and family through life's transitions. As the MSW'S, CRS'S and BH clinicians all come under the behavioral health team as of July, 1, there will be questions during this transition period. But it is important to note that all MSW'S and CRS'S geography and office locations will not change during this transition. This group of clinicians has established solid and important relationships with the clinicians in their respected offices and this collaboration will continue. This is an exciting time for this group of experts and I am very fortunate to be working as part of this team. Advocacy, knowledge, professionalism and community are some of the many attributes of this team. I can only say how lucky we are to have such colleagues at the VNA! The new hire referral bonus program includes employee referrals for RNs, LPNs, PTs, PTAs, OTs, COTAs, and SLPs. If you would like to refer a friend, family member, or colleague, please inform the appropriate Human Resources Representative at your location prior to the applicant coming in for an interview. Please note, when two or more employees refer the same applicant, the referral bonus will be split equally amongst the employees.
Referral bonuses are paid out after the referred employee's three and six month anniversary, as long as they are in a paid status at those intervals. Please contact Cheryl Milas at cheryl_milas@vnacare.org or Debbie Brown at debbie_brown@vnacare.org for more information.

Joint Commission Corner
VNA Care is scheduled for its Joint Commission survey this year, 2016. The team may come as early as July and definitely by December. In preparation for the survey visit we are going to be adding a short informative article in the Connecter each week related to the chapters and or the standards that may come up as part of the survey process. The survey team will be collecting agency data on many aspects of the organization's performance. The survey team will be doing joint visits with selected staff as one of the venues for data collection. One of the great additions we have for our patients is the patient education booklet which has a wealth of patient information related to many topics pertinent to home care services, best practice and improved outcomes. Please take a moment and review the information in the Connecter over the next few months as we prepare for our Joint Commission Survey.
Joint Commission National Patient Safety Goal The Joint Commission standard requires us to identify risks associated with home oxygen therapy. Clinicians are responsible to assess the home for fire safety risks on all patients as part of our environmental assessment. This includes assessing for working smoke detectors and available exits from the home. When a patient is on oxygen therapy we expand this assessment to include proper storage and use of oxygen equipment. It is necessary to identify any person in the home who is smoking or if there are smoking materials in the home. Please review the Patient Education Handout with patient and other people in the home regarding the Safe Use of Oxygen in the home and obtain the patient's acknowledgement of being able to comply with these guideline. This information will be available for all staff and will soon be added to the Patient Education Workbook for patient review. Improper use of oxygen in the home which includes smoking while on oxygen therapy should be reported to you manager immediately so we can assist you to develop a safe plan for the patient, others, and you during the delivery of our service plan.

Q2 OASIS Sessions
This mandatory and relevant 2016 Quarter 2 OASIS Education will take place in each office during the month of June. The focus is Home Health Value Based Purchasing. Discussion will focus on the OASIS questions included in the HHVBP model, and the clinician will have an opportunity to examine outcome, process, and patient satisfaction scores which are part of the model.
