Hi. I just wanted to commend Jerilyn Spokis OT. Yesterday when she was visiting a patient she called me from the home, as pt had HR 160 - 170. Was asymptomatic, but she recognized not to give her a shower, even though the pt. was insisting she have her shower anyway.  She called me, and I was able to move my visit time up to meet her there, so we could collaborate on the pt.  I agreed that HR was correct, and contacted the M.D.  The pt is dehydrated , and we have been watching her closely. She did have to go to the ER and was admitted to Memorial. I haven't heard the final outcome, but the pt. is 90  + yo, so I commend Jerilyn on her great assessment skills, and  also the kindness and concern she shows toward every patient and the families she treats.  I really like working with her, she is a great asset to the VNA and to the communities she serves. thank you. On this particular day, several residents were ill due to a virus.  Also, we had staff that were out sick.  This situation resulted in several residents needing assistance from our nurses and aides.  Many of these residents were not currently on services with our office.   One resident, not on services, asked if we could do a load of laundry for her as she was not feeling well and unable to do it herself.   After asking the aides who could do the load of laundry, Anderson Osei, did not hesitate and volunteered to provide this service even though we were short staffed and he was already doing extra work.  Anderson is always willing to help and assist when needed.  He does a great job.

Susan has always been a great resource to me at New Horizons.  Unfortunately, we had a recent outbreak of a G.I. virus/Norovirus and Susan went above and beyond to assist me with this challenge.  She came to New Horizons to do an onsite review and make recommendations.  Throughout this outbreak, she made herself available by phone and email, setting up tracking grids, researching products and information, and developing new policies.  Susan always got back to me promptly and provided me with the assistance needed.  She made this very difficult process much more manageable.  Thank you Susan.

I would just like to say that Stephanie Premo, CSA in Leominster office, is very friendly and welcoming to all who come into the office. She is quick to answer the door to the office, welcoming to any outsiders and quick in settling them and contacting whomever they need. She also is very conscientious to maintain a neat and professional looking office environment.

I would like to thank Tom Dupont and George Richardson for their efforts during our recent accounting software upgrade. Tom worked extensively throughout the day with the software vendor and our reseller to assure that the installation was complete and functioning.  He coordinated the effort efficiently and kept in constant touch with the finance department providing regular progress updates. There were a few hurdles along the way but the MIS department was able to troubleshoot these issues and achieve resolution leading to a successful install.

I just wanted to say thank you for all your help Friday 3/4/16, with my multiple computer issues. I know it was a 3:15pm phone call on a Friday afternoon with a complicated issue. You were more than willing to help me with a warm  and positive attitude,  and involved a few other members of the IT team to assist. With the end result being, your willingness to meet me in Andover at 5pm Friday with another computer so that I may work this weekend was above and beyond your job requirement and for that I am very appreciative. I was able to connect my computer this morning and start working as all of the programs needed were properly installed on my computer. Thank you again for your friendly and positive attitude to assist my IT needs,

I recently had the honor and privilege of caring for a hospice patient, Donald Hastings.  Mr. Hastings had recently been admitted to a hospital for worsening progression of his disease process and it was clear attempts to "cure" his disease were no longer options.  He and his family opted to allow him to come home for the remainder of his care and elected the hospice benefit.  Although his length of stay with our agency was very brief, the family wanted to acknowledge on particular nurse, Theresa Chagnon, Response Nurse.  During an unscheduled Hospice Skilled Nurse visit that Theresa provided to the home for symptom management, the family would like her to be recognized for, and these are the words of the patients son, Greg, "her ability to guide this ship with gentleness, calmness, witty humor, professionalism and perfection".  He continued to report "our family was in disagreement on the goals of my Dad's care, and she came in here and helped us decided together what was best not only for him, but for all of us.  She made his final days the best he had in years because of her understanding of the entire situation.  We will be forever grateful for her and her visit that day".  I know from having worked with Theresa for the last several years, whether she is our Triage RN, or the Response Nurse providing an unscheduled visit, she is truly an asset to our agency, and the specialty of Hospice care.  She indeed displays her professionalism with her colleagues, and patients and family's we are fortunate to provide care to.  I appreciate her beyond words.  Thank you Theresa.

I'm just emailing to let you know that the Leominster VNA site has a fantastic PT that I shadowed named Kent. I wasn't sure if he's always the one to go out with students, but if it's possible I think he should be. He has a really good way of interacting with patients which was good to see, and it was interesting watching him modify his interventions to work in a home setting.

Welcome to Dr. Joel Bauman

Please join me in welcoming Dr. Joel Bauman to the VNA Hospice and Palliative Care team as Associate Medical Director. 

Dr. Bauman received his BA at the University of Pennsylvania and his MD at the Albert Einstein College of Medicine.  He did his postdoctoral training at Boston City Hospital, the Division on Aging at Harvard Medical School and at Mass General Hospital.  He is a geriatrician and certified palliative care physician who has spent his career in both primary care and specialty practice.

His administrative efforts have included medical staff leadership roles at the Spaulding Rehabilitation Hospital, the MGH Medical ICU Program Project in Palliative Care, and the development of the MGH Geriatric Inpatient Fracture Service in 2010, which was a collaborative effort with the MGH Orthopedic Trauma Service. He continues to provide high quality assessment and collaborative care to a highly vulnerable population of frail elderly patients with major fracture events.
Dr. Bauman's responsibilities will include working with our Palliative Care team, covering our Needham Hospice team, along with the Tippett House and providing on-call coverage.

Droplet
Review of Droplet Precautions
By Susan Stiller, QI Specialist and Infection Control Officer

During the winter months in particular, there are a number of respiratory illnesses in which we want to add additional protection to reduce the chance of contracting the patient's illness. Droplet precautions, using a surgical mask when working close (within 3 feet) of the patient, is to be used for many illnesses such as the flu, pneumonia, and RSV.

We have been seeing more patients with RSV. Respiratory syncytial (sin-SISH-uhl) virus, or RSV, is a respiratory virus that infects the lungs and breathing passages. Healthy people usually experience mild, cold-like symptoms and recover in a week or two. But RSV can be serious, especially for infants and older adults. RSV can be spread when an infected person coughs or sneezes into the air, creating virus-containing droplets that can linger briefly in the air. Other people can become infected if the droplet particles contact their nose, mouth, or eye.

Disease usually lasts less than 5 days, and symptoms are usually consistent with an upper respiratory tract infection and can include a runny nose (rhinorrhea), sore throat (pharyngitis), cough, headache, fatigue, and fever, but some high-risk adults, such as those with certain chronic illnesses or immunosuppression, may have more severe infection, such as pneumonia.

 A surgical or procedure mask should be worn by health-care personnel who are in close contact (i.e., within 3 feet) with a patient who has symptoms of a respiratory infection, particularly if fever is present. These precautions should be maintained until the patient has been determined to be noninfectious or for the duration recommended for the specific infectious agent. When caring for infants ill with RSP, contact precautions should be added as well - a gown and gloves. Educating patients and families to use cough etiquette when coughing and sneezing should also be stressed.

Aligning Annual Competencies Across All Divisions

In 2016 we are aligning annual mandatory competencies that ALL clinical and office based employees need to complete. For employees at VNA Care Network and Hospice, the process will be familiar, however, the content of the competencies have been updated and new content areas have been added to replace retired modules. For VNAB, the process will be familiar but there are additional mandatory competencies so that the agencies can be aligned. For VNAB OFFICE BASED EMPLOYEES AND HOSPICE, and NEW HORIZONS EMPLOYEES the process and the content will be new.

Mantoux
Mantoux Testing in February/March

BEGINNING IN 2016 ALL MANTOUX TESTING WILL BE IN FEBRUARY/ MARCH...MARK YOUR CALENDARS!! Please note that some dates have changed.

Wellness will be doing Mantoux testing for all licensed clinicians around the first quarter OASIS education sessions.
 
This is a change for both VNAB and VNACN.
 
We will be in each office 1-hour before the session starts and will stop 5 minutes before the program begins. We will also be available 1-hour after the program to administer Mantoux. We cannot plant a Mantoux on a Thursday, as staff would have to come in on the weekend to have it read, so I have added additional sessions in a few of the offices that are not around OASIS sessions. Wellness will also be available 2 days after the Mantoux is planted to read the results
 
All licensed clinicians are required to have a yearly Mantoux (or complete a positive reactor's form). Mantoux can be safely given 7 days apart and it is recommended that pregnant women be tested as well.

New HVAC System in Worcester
 
The Worcester office is installing an upgraded heating and A/C system over the coming months.We will need to relocate staff and Facilities and IT are working to get everyone accommodated in temporary locations while their area is being worked on.
 
The schedule is as follows:

  • 1 North (daycare/reception) - 3/14 to 3/18 after hours to accommodate daycare

Employee New Hire Referral Bonus Program

The new hire referral bonus program includes employee referrals for RNs, LPNs, PTs, PTAs, OTs, COTAs, and SLPs. If you would like to refer a friend, family member, or colleague, please inform the appropriate Human Resources Representative at your location prior to the applicant coming in for an interview. Please note, when two or more employees refer the same applicant, the referral bonus will be split equally amongst the employees.

Referral bonuses are paid out after the referred employee's three and six month anniversary, as long as they are in a paid status at those intervals.

INFORMATION SYSTEMS
McKesson
Stability coming for McKesson users

Over the past couple weeks there have been numerous issues affecting the McKesson environment and the use of McKesson on an IPad.  VNA and Atrius IT have been looking into the stability and long term strategy for the McKesson infrastructure as well as the entire VNA environment.  We are taking the following steps to stabilize and eliminate issues that McKesson users are having:
 
  • Prior to the issues there was work already underway to move on to the Atrius Wide Area Network (WAN) which will not only add additional bandwidth but also redundancy so that we can avoid outages where possible.
  • We are also working to make sure that all system related to McKesson are patched with the most current security and operating system updates. 
  • VNA IT and McKesson have also taken a look and allocated storage to support future growth and will be monitoring this ongoing to ensure we do not run out of any storage in the future. 
  • Lastly we are going to replace all IPads with a Lenovo laptops. We had planned to slowly replace laptops over 2016 in preparation with going to Epic. Unfortunately with the recent issues we are going to need to accelerate the roll out over the coming months. Immediately we have stopped rolling out any new IPads. Any new hires will be given and trained on a laptop. We are prioritizing our roll out based on needs and our ability to support those staff on Laptops. Those with IPads should be hearing from one of our IT Desktop Techs over the coming month. We just ask that you be patient as we build and deploy these Laptops as quickly as we can.

New High Alert Medication List Going into Effect

There is a new Foundation policy titled "Medication Management: Orders, Administration, Assessment and Education" numbered VNACN Policy#3.1 and VNAB Policy #5.01.04.  This policy was developed after an extensive review of each agencies policy related to medications. The goal is to have one integrated policy that meets the needs of the patient for safe medication use, along with the regulations/The Joint Commission standards that need to be met. Thus we have one integrated comprehensive policy for Medication management.

There were many similarities in the two policies; one major difference between the 2 agencies is the list of High Alert Medications used by each agency. The new list stated in the policy is a result of best practice taken from The Joint Commission recommendation by the Institute for Safe Medication Practices (ISMP). The list was adjusted to meet the needs of the Foundation. The list is reviewed annually by the medication committee.

This comprehensive list of High Alert Medications will be introduced to staff during the March Team meetings. The effective date for use of the list at VNAB is April 1st 2016. To answer M2010 accurately and to enhance our outcomes for medication teaching this list will be used as the resource for M2010. There will need to be documentation in the clinical record that patient/caregiver was taught special precautions what problems to report, and to whom to report for any medication on the list starting April.  CONTACT PCM WITH ANY QUESTIONS
