I want to give a shout out to all the employees who have helped to welcome and orient our new hires to home care and to the VNA. As of November 2nd, we have hired 321 new employees, 167 certified side clinicians, 46 Hospice employees, 74 office based employees, 18 New Horizons staff, and 4 Children's Garden caregivers. More than 25% of our workforce is new! I join you in welcoming them warmly and enthusiastically.
 
Onboarding more than 300 new employees in less than a year is a phenomenal accomplishment. I want to recognize and acknowledge the care and expertise provided by every preceptor, human resource employee, manager, teammate, coach, and mentor to help our new employees feel welcome and supported as they learn their new jobs and build their knowledge, skills, and confidence. The energy and enthusiasm, skills and new perspectives of our new hires provides hope and excitement, but we could not have brought them on board without your support.
 
As a new hire recently shared,
The VNA is such a special place to work; each employee cares so much about helping patients in need. The amount of work VNA clinicians do on a daily basis is incredible. The amount of care, detail, and empathy that clinicians deliver to each patient is something I have never experienced outside the VNA. VNA clinicians are truly heroes and I have learned so much and value the education I am receiving from them. I am so honored to work with such an amazing group of people.

Good Afternoon Peg, My mom, Isabel C., was recently discharged from VNA/OT care after a hospital stay. I want to tell you that your organization is amazing. I was so pleased with the quality of care my mother received and the attentiveness of her nurse Nicole and her occupational therapist Tom Durkin. They listened to my concerns and provided me with other organizations that might be able to help. Tom was very helpful in getting mom better equipped for showering, safety and assessing her physical strengths and limits. Their compassion and patience with my mom, who has dementia, was welcoming. Diane, a social worker that I had spoken to as well, was so very helpful and sent along a packet of information about dementia and support groups that would benefit me as mom's primary caregiver. As a health care professional myself, I have dealt directly with other nurses from the VNA who have called into my office to give report, and they are just as wonderful. Bravo to your staff. I hope that they are appreciated from an administrative end as much as we appreciate them in the field. 
 
Please continue the wonderful care that you give "our" patients. It's so vital to the patient, the quality of their lives, and their families. Rest assured that if my mother was ever to need your services again I know that she will be in good hands and well taken care of.

I am just shy of my one month new hire and I wanted to send out a BIG Thank you to those who have helped in my transition and training with a special thanks to Beth-Ann Quinlan who is at the top of my list. She has been very helpful, patient and very dedicated in not only keeping up with her workload but taking the time out to assist with any and all my questions. THANK YOU!  I truly enjoy working at VNA Care Network and being part of a great team.

I had the pleasure of receiving a phone call today from a caregiver, Ann, to whom Dana Bishop is the Palliative Care NP for patient, Dolores C.
 
Ann wanted me to know what a wonderful nurse he is and thank him for the empathy and caring he exudes with the patient. He is "sensitive, understanding, kind, and professional." She stated that often people only call to complain but she wanted to call and sing his praises and thank him.
 
GREAT work Dana. You are a wonderful addition to our Palliative Care Team and we are very happy to have you!!

I received a call from a very satisfied and grateful daughter (Michelle) of a patient that we are currently caring for. She spent 20 minutes with me on the phone acknowledging each of the clinicians that have provided care to her mother. The clinicians she referred to specifically were Leah Paquette, RN; Amy Millette, ST; and Maryann Lindgren, OT.
 
Michelle stated that she was also in healthcare and that all of the clinicians exemplified Best Practice with each visit they made. She went on to say, "from a dynamic and customer service stand point Leah provided individual consideration of the patients needs." One of the examples was Leah identifying that the pt was taking a medication that the doctor ordered requiring the pt to take 5 small pills and this could be consolidated into 2 pills with a change in tablet dosage. Michelle states that it was very difficult to get her mother to take those 5 pills along with all her other medications and this made giving her meds in the am much easier. She also stated that when Maryann started with her mother the patient had no use of right arm and that recently her mother proudly showed her that she could put a K-cup into her coffee maker and make herself a cup of coffee using her right arm. Michelle stated that Amy has also worked miracles with her mother's language skills.

In Honor of National Home Care Month
 
The VNA is currently offering clinical learning experiences in home care practice. This year alone we have provided experiences for to 21 Physical Therapy students and more than 70 nursing students. In honor of National Home Care Month, we asked the some of these students to share with us their impressions of home care practice. Here are some excerpts:
 
Home care has been much more of an eye opening experience than I thought it would be. I never realized how little someone can know about their own medications, treatments and aliments. I really enjoy that in home care, we are able to be with the same person weekly. I feel like it makes the experience much more personal and I really feel as if I am helping someone and I can see the interventions I am teaching working.
 
I see the place of home care as more than an extension from the hospital. I think it is great that home care clinicians are able to go into the home of the patient and ensure that they truly understand what was taught to them on discharge and be able to keep a more watchful eye on the patient. This watchful eye and additional teaching are a great combination in preventing the patient from returning to the hospital, or experiencing complications from lack of knowledge.
 
Home care offers the patients someone that is really able to come in and assess what is going on at home. Just because your patient tells you that their house is spotless and there is no risk of them tripping, does not mean it's true. A cardiac patient can say they understand the SALT diet and follow it, however, it's the home clinician that really gets to see and teach the patient what to look for in the ingredients to ensure they are understanding the diets they should be on. I believe that home clinicians also offer a different sort of relationship with the patient. Not being in the hospital allows for a relationship where you are able to get to know them as a person more than just a patient.

Staff Flu Clinics

Alright people, we're getting there! If only 400 or so of you get your flu shot (or provide a waiver or proof of receiving the vaccine at another, non-VNA location) we can reach our goal of 100%!! No big deal.

 It is mandatory for all staff to be immunized against the flu. If you receive the vaccine at another, non-VNA location, you must show proof of receiving the vaccine to be in compliance with the VNA's policy.

Questions from last week
  1. What does VIS mean on the flu consent form? Answer: Vaccine Information Statement, which is the CDC leaflet which needs to be explained to each individual receiving the flu shot. The date given is the same as the date the shot was administered IT MUST BE FILLED IN.
  2. What is the MIIS, in relation to flu? Answer: The MIIS is a new (mandatory 2015) statewide system to keep track of immunization records for individuals receiving flu vaccine.  Thus, the need this year for the second signature which MUST BE FILLED IN.

Note that TB testing is available for VNACN staff at some flu clinics as well as during Annette Benoit's team meetings in Worcester on 12/3 and 12/10 from 8:30-10:30am.

Telemonitoring
Telemonitoring Update
 
The Telehealth Department is quickly growing its census and moving toward maintaining best practice and working on collaborative initiatives to continue to reduce our hospitalization rate.

To date, there are 250 VNACN and 93 VNAB patients on telemonitoring devices...great work everyone for referring patients and working together in the care of these patients!

We have 22 Remote Patient Monitoring people on monitors, these people do not meet skilled criteria, and have a critical need for daily monitoring, thus we are fortunate to work with our Atrius partners to monitor these patients daily under contract. This is another innovative program utilized within our agency.
 
In an effort to maintain consistent vital sign readings, we ask that when a patient has a monitor in the home that all clinical staff that take vital signs on these patients to please use the monitoring device equipment to maintain a consistent cuff size. This will add to accuracy of the vital signs.

Alcohol Prep Pad Conversion

Conversion to Alcohol Prep Pads for Disinfecting Patient to Patient Equipment 
The certified side of the VNA is in the midst of converting to large alcohol prep pads for disinfecting equipment that clinicians use patient to patient. Alcohol prep pads are to be used to disinfect equipment such as BP cuffs, stethoscopes, and pulse oximeters after patient use. The large pads are sufficiently saturated with alcohol to moisten the entire part of BP cuffs that comes into contact with patient skin. The prep pad needs to be open to obtain full value of the size.
 
We recently performed blotter paper tests of several types of alcohol prep pads and the McKesson product we are now stocking proved to have the best saturation of alcohol.
 
Alcohol prep pads provide sufficient disinfection for most equipment. However, any equipment that involves blood and body fluids must be disinfected with a bleach pad. Any equipment used with patients who have (or who you suspect may have) C. Diff must also be disinfected with bleach wipes.
 
We are investigating best wipes for our MCH nurses to use in cleaning baby scales. Stay tuned for more to come on this practice issue.

Facilities update for this week:
  • All buildings are being prepared for the fall and winter seasons.
    • The Irrigation company will be at the deRham home on 11/5 and the Tippet home on 11/6 to blow out the sprinklers
    • All HVAC systems have been winterized as of mid-October.
  • Rose Monahan Home: renovations to the exterior decks are to be completed by the end of the week. Planning for renovations to the interior of the home has begun.
  • Tippet Home: A new stove has been delivered and installed
  • Leominster: signage being put up on doors by mid-November
  • Worcester: window blinds are being installed in addition to paint and carpet touch ups. Furniture in the HR department will be delivered/installed the week of November 16.
  • Woburn: Planning has begun for an HVAC unit in the server/data room
  • Rug cleaning will soon begin (weather permitting). Please be aware that common areas and walkways are to be cleaned. If your office or workspace has a stain that needs to be cleaned, please contact Tim Anderson at 508-756-7176 X6961 or tanderson@vnacarenetwork.org.

Employee Referral Bonus Program Expanded to Include LPN, PTA, and COTA!

The referral bonus program has been expanded to include employee referrals for RNs, LPNs, PTs, PTAs, OTs, COTA's, and SLPs. If you would like to refer a friend, family member, or colleague, please inform the appropriate Human Resources Representative at your location prior to the applicant coming in for an interview. Please note, when two or more employees refer the same applicant, the referral bonus will be split equally amongst the employees.

Referral bonuses are paid out after the referred employee's three and six month anniversary, as long as they are in a paid status at those intervals.

IS Update
 
Updates from the IS Department
  • We have been working with Atrius to put a faster and more resilient network in place between all of our offices. All of the offices will have upgraded bandwidth (bigger pipes) and backup connections from separate carriers that will automatically take over for network traffic if there is a problem with one of the lines. We have started to do site visits with one of the carriers to enable them to get us connected, those sites include: Leominster, Danvers, Southborough, and Woburn. We will keep you updated on the progress.
  • We have recently deployed more than 200 new laptops to replace aging devices in the field.
     
  • IS continues to work on the plan to update and replace the network equipment in the offices. We are close to finalizing the configuration and hope to have specific dates for each office soon.
  • The team assessing the ShoreTel to Avaya migration for the Charlestown, Quincy, and Woburn offices continues to meet and identify all areas that need to be considered as we look to get everyone on one phone system.
  • In an effort to improve clarity in its communications, the IS Department will now present its on-call contact information in a simplified form. With that, here are the IS off-hours contacts for this week:

SERS Update

A great big thank you to all staff you have been using the SERS reporting system over the past few months...your input and due diligence has been awesome and most appreciated. With the data that we have been able to track via the new system, we are able to easily evaluate for trends and start to put some improvement plans in place.
As you can see from the above graph, our largest opportunities are Falls reduction, Communication and Service, and Information Technology. More detailed information found in the Falls category can be found below.
 
We are working on a plan of action to reduce falls, which will be rolled out in the next quarter under the direction of Judy Walsh and the Rehab Department. More to follow in the Connecter, please stay tuned for this education and best practice.
 
A few reminders for SERS entry:
  • Please do not add people's names in the narrative section, use clinician, etc
  • There should always be some notation of follow up as to what was done to educate, improve, etc.
  • If staff entering SERS can avoid using "other" that will be helpful, if you find you cannot find a category please notify me and we will most likely be able to add a drop down for that category.
If anyone still needs SERS training, please contact your office's Patient Care Manager (PCM) and they will assist you with learning how to enter an event. If you cannot find the SERS icon on your iPad, please contact IT and they will assist you. There have been a couple Connecter articles regarding how to find/use the SERS icon on iPad, reviewing the archives may be helpful.
 
The agency has done a fabulous job in engaging in this new event reporting process, thank you so much for all your help.

At SOC, Mr. Ford noted that his left hip was so uncomfortable that he wakes suddenly at night during any position changes. He also notes that throughout the day he has trouble getting into a comfortable position and he hesitates to get out of his recliner as often as he should. You spend time educating Mr. Ford about his prescribed medication regime that includes Tylenol™, Tramadol & Oxycodone. How would you answer the following OASIS question at SOC?
