I am always so proud of our team, and I know that for every call I get, there are 100s of patients and family members who feel this way and don't think to pick up the phone. When they do, I want to be sure that I share this. Thank you all for everything you do each day for our patients.

SERS Compliments/Complaints Update

Ailish from Atrius and Karen Chirsky are working to get an icon for complaints/compliments into SERS, but in the meantime, please note that these events still need to be completed on paper forms.
If your complaint is also an event, such as a delay in service accompanied by a call from referral source of patient complaining about the delay, add into SERS and it will be manually sent on the back-end to Karen Chirsky in Business Development for follow up.

Staff Flu Clinics

It is once again mandatory for ALL staff to be immunized against the flu.
IF you are not immunized you will be required to wear a mask while you are working during the entire flu season; each day from October 1st through the spring of 2016.
IF you receive the vaccine at another location, you must show proof of receiving the vaccine to be in compliance with the VNA's policy.
If you are unable to receive the vaccine, it must be documented, and sent to wellness. Valid reasons are: - Religious exemption (you must speak with your human resources representative).
- Medical reasons (you must have written note from your physician)
352 staff received their flu vaccine last week, let's keep moving towards our goal of 100%!

Behavioral Health Update

As Behavioral health continues to grow, we wanted to answer some frequently asked questions.
1. When should I make a referral to Behavioral Health?
It is important that a patient be medically stable prior to making a referral to Behavioral Health. Often, clinicians are helping the patient in a variety of ways, initially, and the patient can be quite overwhelmed with the number of visits and different clinicians coming in and out of their home. So, we ask that Behavioral Health be considered when visits are slowing down and the patient is feeling more comfortable medically. In this way, we find the patient is more often receptive to Behavioral Health as they are having fewer visits and fewer appointments to manage. A patient must have a documented diagnosis of depression, anxiety or early onset dementia.
2. When I make a referral to Behavioral Health as the primary clinician, can I automatically discharge the patient from my services?
In preparation for a referral to Behavioral Health, we ask that you refer to Behavioral Health at least two weeks prior to a planned termination of services. In this way, the Behavioral Health Clinician has time to go out and evaluate the patient for the appropriateness of the program. If the patient is not appropriate for Behavioral Health, this gives the primary clinician time to possibly refer to other potential services. If the patient has been determined by Behavioral Health to be appropriate for the program, the Behavioral Health Clinician will contact the primary clinician to work out termination of the primary's services. Please do not terminate services automatically once you have made the referral to Behavioral Health. We know this can be confusing as Behavioral Health Clinicians are RN'S but an evaluation must be performed and a determination made.
3. When I am getting ready to discharge the patient as the primary clinician and Behavioral Health is now working with the patient, should I discharge the patient?
We have several cases where the SN, PT and OT are working alongside the Behavioral Health clinician. When the primary is preparing to terminate their services, please do not discharge the patient from VNA services. We ask that the primary clinician and the Behavioral Health clinician work together coordinating a smooth transition to Behavioral Health as primary. This coordination would involve the primary clinician completing a discharge summary and then transferring the case to Behavioral Health as well coordinating a possible transfer in the midst of a recert window.

Infusion Update

Please be aware that the Beth Israel Deaconess Medical Center Facilities are changing the ambulatory pumps used for chemotherapy administration in the community. Previously they used the Walkmed, they are now using the CADD Legacy. We have a trainer in the Worcester office and any member of the IV team is able to provide a quick tutorial.
Instructions are simple: press and hold the stop button; disconnect tubing from patient; cap tubing with cap form saline flush; flush port per policy and dispose of Huber needle in sharps container. Remove battery, remove tubing; place IV bag, tubing, and gloves into chemo bag, zip closed and place inside second chemo bag. Send pump back to clinic with chemo bag, not inside chemo bag.
Also, you will see several new items in your EMR related to IV Event, and Document Communications, depending on if you are McKesson or Encore. Please ignore them. They are a new tool the MIS department and I are using to validate the excellent care you are providing. Our goal is to be able to produce statically relevant information linked directly to our EMR. Please bear with us. We are working to demonstrate how awesome you are.

Best Practice Committee Monthly Update

The VNAB is committed to the goal of reducing our rehospitalization rate. The agency's goal is to have all patients admitted by nursing to have their 2nd nursing visit within 48 hours of start of care. We now have a year's worth of data so I will be reporting cumulative data for a 12 month time period.
For the time period September 1, 2014 to August 31, 2015, the agency's rate for having the second nursing visit within 48 hours of start of care for high risk patients was 68%. This is the same as the previous timeframe.
We are also promoting two additional interventions that are best practices for keeping patient's from being rehospitalized.
- The first best practice intervention is that continuity of care with providers improves patient outcomes. To support this practice we will be reporting on the percentage of patients who received their 2nd nursing visit by the nurse case manager. The rate for the time period of September 1, 2014 to August 31, 2015 was 49% which is higher than the rate for the previous time period.
- The second best practice intervention is that the patient have a physician follow up visit within 7-14 days after a discharge from an acute care hospital. The rate for the time period September 1, 2014 to August 31, 2015 was 74% which is the same as the previous time period.
Thanks for your continued efforts to keep patients out of the hospital particularly during the four weeks after they first come home from the hospital. The next report will be for the time period of September 1, 2014 to August 31, 2015.
Please remember that a great way to help keep our patient's out of the hospital is to provide telemonitoring for the high risk for rehospitalization patients. We now can provide monitoring for our COPD patients in addition to our Heart Failure patients.

Annual TB Testing
VNAB employees who have patient contact are required to be screened for TB annually. The schedule for this year's Mantoux testing is below.
You must attend one of these sessions and have the test read within 48-72 hours of the planting or you will be out of compliance with state and federal regulations. Consequently, you will not be eligible to work in the field. Please select a time and plan to attend that session. Also, be sure to schedule the time you will need to have the test read (48-72 hours after screening date). It must be read by a VNAB Nurse Manageror Wellness Nurse.
If you have had a previous test in 2015, you are still required to have another one planted in September in order for us to keep all employees on the same cycle. If you tested positive in the past, you will receive an email with a questionnaire to complete and return to Human Resources before September 30, 2015.This too is a job requirement.
Facilities staff will be present at the screenings to calibrate your blood pressure cuffs, pulse oximeters, FOB's, and check/update cell phones and beepers. Please bring your BP cuff, pulse oximeter, FOB , and cell phone to the screening.

Upgrade to Encore Clinical and Financial Tuesday, September 22

This upgrade is primarily for ICD-10 purposes. Please read this message carefully.
- Encore Clinical and Financial will be down beginning at 5:30 AM on Tuesday September 22
- Clear all Work "Saved in Process" by the end of the day Monday September 21
- Log off of Encore Clinical and Financial at the end of the day Monday September 21
- Triage: Log off at 5:30 AM Tuesday morning
- Do not attempt to log into Encore Clinical or Financial until an announcement is made via Outlook email by the MIS Department that the upgrade is complete. (After 8am) Attempts to log on prematurely will cause delays in the upgrade and result in rework by MIS.
- You are free to use Outlook or any non-Encore application as you would normally.
- Team schedulers will not have access to Encore Financial team Scheduling until after the upgrade is complete. Please schedule everything possible Monday afternoon so that your clinicians pick up assignments overnight. Laptop communications cannot be performed until the server update is complete and an announcement is made.
- Print team schedules on Monday afternoon
If you experience issues after the Code Load please report them to your office's PC Tech.

Final Reminder: McKesson System Upgrade the night of September 28

A McKesson upgrade has been scheduled for the night of Monday, September 28. The upgrade will fully prepare us to start using ICD-10 codes. Any other changes will be communicated in a separate Connecter article. These are the details regarding the upgrade:
Here is what you need to know about the night of the upgrade:
- The system will be shutdown at 9pm sharp on Monday, September 28
- During the shutdown the following systems will be unavailable:
- Horizon Homecare for VNAB & Hospice
- TeleHealth Interface
- WebChart
- Web Referrals
- Laptop Users:
- Full detail and instructions regarding the ESD transfer will be communicated in an email.
- Please be aware that the ESD transfer will need to be performed in the office via network cable and will take about 30-45 minutes. Further detail to follow.
- iPad Users and Office Staff:
- You must be signed out of the system by 8:45pm to avoid losing any documentation
- Please do not log on again before 6am the next morning

- It has been brought to Facilities' attention that staff have been missing items from their workspace and food from the kitchen refrigerators. Please respect each other's property and make sure to label food left in the refrigerators.
- Your ID badge is now your key access to the sites that you are assigned to. If you have any questions please email Lorraine Potter or Timothy Anderson.
Charlestown
Building management has seen an increase in the number of bicycle locks left overnight on the bicycle racks located around the building. Maintenance personnel will begin removing these locks each evening, so please make sure not to leave them behind.
Danvers
American Alarm installed the new ID badge system on Monday, September 21. If you have not already received your new ID, please see the receptionist.
Gloucester
Due to the front door not closing tightly, we are adding an automatic door closer. This will only aid in ensuring the door closes tightly and your existing key will still work in the lock.
Leominster
An interior folding wall is being installed this week.
Needham
New ID badge system has been installed. Access was provided to weekend staff over the weekend. If you have not already received your new ID, please see the receptionist.
Woburn

Step Out for Hospice this Sunday
Grab your sneakers and join us for the 18th annual Step Out for Hospice in Woburn on Sunday, Sept. 27, to benefit VNA Hospice Care.
Register for the 3-Mile Walk or 5-Mile Road Race by Sept. 23 for a discounted fee of $30. You'll also be guaranteed a Step Out for Hospice sweatshirt. Step Out for Hospice starts at the Irish American Club of Woburn, 147 Main St., and goes around Horn Pond. Children ages 13 and under can join the 1-mile Fun Run beginning at 11:30 am. The Fun Run is free. Prizes and a limited number of T-shirts will be available for Fun Run participants.
The 3-Mile Walk starts at noon, and 5-Mile Road Race at 12:30 pm. Participants are invited to enjoy the post-walk/run festivities, including live music and dancing, chowder, hotdogs, and raffle.
Step Out for Hospice is being organized by volunteers from Woburn Friends of VNA Hospice Care, the Irish American Club of Woburn, and the Shamrock Running Club.

Cool Hot Jazz this Friday Benefits Home Health and Hospice Care
| Henri Smith New Orleans Friends and Flavours are on the bill for Cool Hot Jazz, a benefit for VNA Care Network and VNA Hospice Care, on Friday, Sept. 25, at 6 p.m. in the Needham Golf Club, 49 Green St., Needham.
The evening starts with cocktails, heavy hors d'oeuvres and silent auction at 6 p.m. Henri Smith New Orleans Friends and Flavours will perform at 7 p.m. The evening also features dancing, live auction and cash bar. Henri Smith thrills audiences with his sophisticated and dramatic vocal delivery of jazz, blues, Creole, and Cajun flavored music, touring extensively throughout the United States and Europe. He frequently performed and served as the emcee at The New Orleans Jazz and Heritage Festival, the largest Jazz festival in the world. Hurricane Katrina took Smith's home in New Orleans, so he now lives on Cape Ann, where he shares his Creole and Cajun heritage. |

iOS 9 Update on Apple devices
This message is intended for anyone that has been issued an Agency iPhone or iPad.
Please ignore any requests by your devices to upgrade to IOS 9. Doing this upgrade may and probably will wipe out the configuration settings for access back to any Email and Connectivity currently configured on your device.
If you inadvertently do or already have and are having issues, please reach out to your local IS resources for assistance.

Update to VNAB Outlook Email
The IS Department will be performing an upgrade to the VNAB Exchange servers on Wednesday, September 30 starting at 8pm. During this time, you will not be able to access your VNAB email. This includes:
- Outlook (desktop, laptop and Citrix)
- Mail (iPad and iPhone)
- Webmail
We expect the work to be completed by 12am. Please be sure to logout of Outlook prior to Wednesday at 8pm to ensure the update completes on your machine. If you have any questions, please contact the VNAB Help Desk at 617-886-6822 or X6822.

Quarter 1 OASIS Make-Up Reminder

For all clinical staff:
Quarter 1 OASIS make-up information packets need to be completed and returned to your manager by October 1, 2015. You should have received them approximately one month ago or longer. If you were unable to attend OASIS Q1, then you need to review the packet, submit to your manager; QA will review and then remediate with you one to one what you need assistance on.
OASIS is the document that drives the patient care plan outcomes. It is important that all staff understand the OASIS questions and rational for each answer. The OASIS education in-services are very, very helpful in increasing knowledge base around OASIS - the sessions are interactive and a ton of fun! If you are able to attend in person, which is the best venue for learning, you do not need to complete a packet. If you did not attend in person, please complete the packet ASAP (but definitely by October 1) and get it back to your manager.

Change #4 - Referral - Demographics / Clinical Forms / Notes

Earlier this year, multiple clinicians suggested documentation improvements in Encore. These clinician suggestions lead to the formation of the Encore Documentation Reduction Committee. Over a period of several months the committee, consisting of a group of multidisciplinary clinicians and managers consulting with their peers, was charged with reviewing Encore Documentation standards and developing solutions to meet committee goals of:
- Simplifying Encore Documentation
- Removing Redundant Encore Documentation where possible
- Streamlining Encore Documentation and processes wherever possible
The third area of focus for the Documentation Reduction Committee involves clinician documentation when entering an assessment.
Please see below for pictures and details for each change.
Reminder, patient diagnoses listed by the Referral Center will include diagnoses and history which have the potential to impact the Plan of Care including outcomes. Initial OASIS - M0102 & M0104 (see picture below) This change is specific to first clinician completing SOC or ROC. Complete only ONE of these "M" items (once one is completed, the other will be dithered and therefore inaccessible to documentation).

Q3 & Q4 OASIS Education Sessions
The following are the scheduled Q3 AND Q4 OASIS Education schedules. All clinical staff are mandated to attend 1 OASIS Education session per quarter, this includes RN (staff and contract), LPN, clinical managers , PT and PTA's, and clinical coordinators. Quarter 3 reviews Functional OA information and Quarter 4 will review miscellaneous OA education and COPS.
The QA Department has scheduled out many sessions in all offices for the final two quarters of the year to accommodate multiple offerings at multiple sites. Please work out the time that you choose to attend with your individual manager, you may attend at any site.
Staff must attend ONE education session per quarter. Our goal is to maintain our current best practice and strive for the top 20% in our reportable outcomes...remember that we are at 4 Stars for VNACN and 4.5 Stars for VNAB, with a goal of 5, we would like to keep the momentum going!!!
