As our service standard implies, anticipating our customers' needs is our priority.
I have been working closely with one of the nurses covering the Roxbury census track. Recently we had a patient on caseload that was referred to the agency due to unexplained weight loss.
Initially, Michelle, the primary nurse, and myself were unable get in touch with this patient as his phone was not in service and he would not answer his doorbell. Upon admission it was noted that patient required additional services and resources, he was missing medications, not enough food at home, and the patient also needed a lot of cleaning done in his apartment.
I was able to communicate with the patient's emergency contact, also communicating with the nurse at the primary care doctor's office, primary nurse, pharmacy, and the patient's homemaker.
The patient now has a working phone, his medications are now pre-filled and delivered monthly, and he is now able to verbalize his medication schedule correctly. The patient now has a working scale at home to keep track of his weight, meals on wheels, transportation to his doctor's appointment, homemaker services in place for ADL's and IADL's five days a week, as well as laundry services.
Our nurses can now reach the patient for his home visit appointments. He keeps his follow up appointments and adheres to his medication and dietary regimen. The patient's apartment has been cleaned. The patient is very pleased and grateful for all the hard work by such great team of health care providers. I must say that I am very proud to be a part of such an organization where we do not just anticipate our customers' needs but we follow through and deliver exceptional services to our customers at all times.

Michele Flanagan, RN in the Worcester office shines the spotlight on Maryam Lempke, RN
I am currently precepting Maryam Lempke, RN in the Worcester office. Last weekend, while making calls to arrange visit times, a patient's mother answered the phone and asked Maryam about her daughter's condition. The patient is a grown woman who lives with her mother. Maryam explained to the patient that she could not give this information to her. When we arrived at the home, the mother answered the door, and again asked about her daughter's current health condition. Maryam explained to her that this information is private, and talked to her about HIPAA and why she cannot give this information. We later found out that the patient and her mother do not have a healthy relationship, and the patient was very grateful that her health information was not shared with her mother. Maryam was able to stay calm while the patient's mother continued to badger her for information. She was able to explain the patient's rights to privacy in a professional manner. Great job!

Staff Flu Clinics

Some good news!!! For ALL employees and (homebound) patients on-service, the agency was able to secure the flu vaccine very early this year. Remembering, the sooner you are immunized against this year's strain of flu, the better your chances of NOT becoming ill. It is once again mandatory for ALL staff to be immunized against the flu. IF you are not immunized you will be required to wear a mask while you are working during the entire flu season; each day from October 1st through the spring of 2016.
IF you receive the vaccine at another location, you must show proof of receiving the vaccine to be in compliance with the VNA's policy. If you are unable to receive the vaccine, it must be documented, and sent to wellness. Valid reasons are: 1) Religious exemption (you must speak with your human resources representative). 2) Medical reasons (you must have written note from your physician)

Everyone should have the SERS icon on his or her device. There are instructions in last week's Connecter with instructions on adding the SERS icon to your iPads Citrix tray. Please review and make sure that your icon works, you need to log in choosing the site you are from VNAB or VNACN, your user name and your password that is used for you other log ins.
If you cannot access SERS, please notify the PC Tech in your location or contact another member of the MIS Team.
For those who attempted to attend last Thursday's in-person training session in Weymouth, please accept my apologies for the confusion. Ailsish, the Atrius representative conducting the SERS training for all VNA staff, got stuck in severe traffic on 93 and, while she made every attempt to get to Weymouth in time for the training, it was out of her control and (as those who were there know) she never made it to the office. If you are unable to attend one of the additional training options listed above, please contact Donna Peters at dpeters@vnab.org.

New Chief of Pulmonology at Atrius Health
I am pleased to announce the hire of Dr. Adam Smith as the new Chief of Pulmonology at Atrius Health, with an anticipated start date of November 1, 2015.
Dr. Smith joins Atrius Health following his role as Chief of Pulmonology at South Shore Hospital. He has been in clinical practice at Harbor Medical Associates and South Shore Hospital since 2010. Dr. Smith has devoted a great deal of his office practice towards caring for patients suffering from COPD (chronic obstructive pulmonary disease). In the hospital setting he has spent time working in the ICU and consulting on pulmonary patients. He has also developed a disease management program aimed at reducing COPD exacerbations. Dr. Smith hopes to bring a holistic approach to the care of patients with respiratory conditions, addressing the comorbidities, such as anxiety, depression, and nutrition that play a major role in the care of pulmonary patients. He also has a clinical interest in lung cancer diagnosis and staging, as well as the care of patients with interstitial lung disease. Dr. Smith will be practicing at our Harvard Vanguard Kenmore and Somerville offices.
Dr. Smith graduated from NYU School of Medicine. He then completed a residency in internal medicine and pediatrics and a fellowship in pulmonary and critical care medicine at St. Vincent's Hospital in New York City. Please join me in welcoming Dr. Smith to our practice.

McKesson System to be Upgraded Monday, September 28th

A McKesson upgrade has been scheduled for the night of Monday, September 28th. The upgrade will fully prepare us to start using ICD-10 codes. Any other changes will be communicated in a later Connecter article. These are the details regarding the upgrade:
Here is what you need to know about the night of the upgrade:
- The system will be shutdown at 9pm sharp on Monday, September 28th
- During the shutdown the following systems will be unavailable:
- Horizon Homecare for VNAB & Hospice
- TeleHealth Interface
- WebChart
- Web Referrals
- Laptop Users:
- Full detail and instructions regarding the ESD transfer will be communicated in an email.
- Please be aware that the ESD transfer will need to be performed in the office via network cable and will take about 30-45 minutes. Further detail to follow.
- iPad Users and Office Staff:
- You must be signed out of the system by 8:45pm to avoid losing any documentation
- Please do not log on again before 6am the next morning

It's Annual OSHA TIME!
Fall marks the time when all VNAB clinicians, clinical managers, liaisons and anyone making patient visits needs to complete the mandatory OSHA competency. There is a new module this year with new information about infection control and work place safety. Employees will have from September 21st to October 30th to complete the module and pass the accompanying quiz with a score of 80% or higher.
The module is new so it is recommended that you go through it with care. You are given 1 hour of meeting time to complete the module and the quiz.
THIS YEAR VNAB EMPLOYEES WILL TAKE THEIR COMPETENCIES ON THE NEW EDUCATION PORTAL WEBSITE, instead of on Home Care Central. THE EDUCATION PORTAL IS AN EASIER TO USE SITE AND IT IS NOW THE EDUCATION WEBSITE THAT IS SHARED BETWEEN VNAB AND VNACN. ALL CONTENT, INCLUDING MANDATORY COMPETENCIES, IS NOW LOCATED ON THIS EDUCATION PORTAL.
To access this new site, go to the staff page on the VNAB website. Scroll down below the Harpers/Payroll links and find the link for the EDUCATION PORTAL. Click on this link and you will come to the sign in page. The first time you access the site you do need to set up a password. Instructions for doing this and then taking the OSHA module are in the pdf below.
This pdf is also posted beside the link to the Education Portal.
We encourage you not to wait until the last minute to complete your OSHA competency as the process is different and you don't want to be rushed. If you follow the instructions you will be fine.

Patient Discharge Instructions

In the upcoming weeks, you will notice an addition to the patient handbook; a new discharge instruction handout has been developed for use by VNAB and VNACN clinicians. As patients are discharged from our agency, it is expected that they (and/or caregivers) have the knowledge and tools to self manage their disease process to the best of their ability. However, it may be difficult for them to remember all of the important information that was provided over an entire episode of care. The new discharge handout can help.
To promote interdisciplinary care, there are 2 sections, "Health Management" and "Function and Mobility." Clinicians should fill out relevant and appropriate information in both sections, regardless of discipline. If the case is multi-disciplinary, all clinicians can contribute by adding necessary information at their discipline's discharge. Lastly, the discharge summary should be easily accessible to the patient. Consider taking it out of the handbook and placing it on the refrigerator within clear view.
It is important that our patients know that although they are being discharged from our agency, they have the knowledge and information necessary to self manage their disease process, stay safe at home, and stay out of the hospital!
Your PCM will be providing more information on how to fill it out at upcoming team meetings.

Important Notice to All VNAB Staff with Patient Contact | Annual TB Testing
VNAB employees who have patient contact are required to be screened for TB annually. The schedule for this year's Mantoux testing is below.
You must attend one of these sessions and have the test read within 48-72 hours of the planting or you will be out of compliance with state and federal regulations. Consequently, you will not be eligible to work in the field. Please select a time and plan to attend that session. Also, be sure to schedule the time you will need to have the test read (48-72 hours after screening date). It must be read by a VNAB Nurse Manageror Wellness Nurse.
If you have had a previous test in 2015, you are still required to have another one planted in September in order for us to keep all employees on the same cycle. If you tested positive in the past, you will receive an email with a questionnaire to complete and return to Human Resources before September 30, 2015.This too is a job requirement.
Facilities staff will be present at the screenings to calibrate your blood pressure cuffs, pulse oximeters, FOB's, and check/update cell phones and beepers. Please bring your BP cuff, pulse oximeter, FOB , and cell phone to the screening.
Thank you for your cooperation in meeting this important job requirement. It is designed to protect your health and the health of our patients.

Encore Codeload/Software Update: Tuesday September 22nd
Instructions for laptop point of care users. Please read carefully as these instructions are very important.
Set your laptop up for a laptop synch Monday night the 21st even if you are not working Tuesday, if at all possible. Communicate the Revision file to your laptop immediately if you have not already done so. The Codeload goes live at 6:00 am on Tuesday, September 22nd.
Now:
- Clear all (H) Held Email
- Clear everything out of "Complete Work in Progress"
Monday, September 21st:
- Do an immediate synch to send over your work once finished.
- Do not enter anything including Encore email after this Communication
- Set up for your usual nightly Laptop Synchronization
Tuesday, September 22nd:
- Do not touch your laptop at all until 06:30 am or later.
(Office staff: the server will not be ready until after 8am, wait for announcement by email.)
- Then, clear the holding screen or turn your laptop on and click into Encore.
- If you see a lot of technical jargon scrolling on the screen, do not interrupt this process. This is normal and will clear in approximately 30 to 45 minutes. Look for a successful revision message.
- If you do not see a lot of technical jargon scrolling on the screen, check the version of Encore in the upper left corner of the screen. If the version is 06.00.0010, you are all set. If you do not see this version, log off and shut down your laptop. Then, log back into Windows and Encore. You should see the scrolling of technical jargon begin. Remember that it may take 30 minutes to complete.
- You may use your laptop normally if the version is correct and you have no other error messages.
- Do not communicate your laptop until we make an email announcement that the communications are functional.
Purpose: Preparation for Changes to the ICD-10 codes.
Common Problems:
- Revision file never communicated to the laptop-your laptop is unusable, turn it in
- Did not finish and communicate work by Monday night-it may be lost and need to be recreated
- New version not updating-try logging out of Encore or into Encore
- Any other error-request support
Prepare ahead as the server will be down for about 2 hours early on Tuesday. Ask for printouts Monday of any forms i.e. Med List that you would need to provide patient care on Tuesday should your laptop fail.

We are changing our document destruction/shredding vendor from Cintas to Universal. New containers will be arriving at all offices, but the pickup times and process remains the same.
Charlestown Window washing taking place Thursday into Saturday. Please have all sills cleared off as they will be cleaning both interior and exterior windows.
Danvers
American Alarm will be installing the new ID badge system on Monday, September 21st at 7:30 a.m. Receptionists will distribute new IDs.
Needham
American Alarm will be installing the new ID badge system on Thursday, September 17th at 7:30 a.m. Receptionists will distribute new IDs.
Woburn
New ID badge system has been installed. If you have not already received your new ID, please see the receptionist. New cubicles will be installed Wednesday, September 16 in various areas throughout the office.
Worcester Fire detectors will be in testing mode on the morning of Wednesday, September 16 while ceiling tiles are replaced around the smoke detectors.

Upcoming Cool Hot Jazz Benefit Featured on Needham Channel News
Henri Smith New Orleans Friends and Flavours will perform during Cool Hot Jazz, a benefit for VNA Care Network and VNA Hospice Care, on Friday, Sept. 25, at 6 p.m. in the Needham Golf Club, 49 Green St., Needham.
Jane Woodbury, vice president of fund development, and Louise Condon, a volunteer with the Needham Area Community Cabinet, were interviewed by Needham Channel News about the benefit and our home health and hospice services. Watch the video at: http://bit.ly/1ODNKge

Patients and Families Honor Staff through Thanks for Caring Society
Often, patients and family members make donations in appreciation of the exceptional care and service provided by our staff. These donors become part of the Thanks for Caring Society, and we'll recognize the staff they've honored each month in the Connecter and annually in the donor report.
These types of donations are received directly from patients and family members and as a result of direct mail appeals from the agency. Donors who make gifts in honor of staff will become part of the Thanks for Caring Society. For all staff who are recognized in this way, the Fund Development team notifies the staff person and his/her manager that a donation was made in honor of the care provided. If multiple staff members are honored, each is notified; if a team or group of providers is honored, the manager is notified on behalf of the group. If the donor includes a note with their gift, a copy is included with the notification letter to the manager.

IT/System Update
Below you will find a facility update by office of what's happening over the next several month. Please contact your office's IT representative with any questions.
All Sites:
- Researching, testing, and validating Atrius "free" vs. "busy" on Outlook calendars. This will at least enable meeting schedulers to determine openings on Atrius user's calendars.
- Researching, testing, and validating mobile device management (MDM). This is an expansion of our mobile device hardware already installed.
- Working on upgrading the entire computer network speed and capabilities
ShoreTel to Avaya: Impacting Charlestown, Weymouth, and Woburn Staff
Team is working on changing the ShoreTel telephone system over to the Avaya system. There are many benefits included in this change, with the biggest plus being 4-digit dialing between all offices. A committee of impacted staff is being established to ensure a smooth transition to Avaya. If there are certain functions of the ShoreTel system that you want to make sure are considered, please consider joining the committee or contact George Richardson with your input. Goal is to have the Avaya phone system in place organizational-wide by the end of the year.
Planned downtime
- Encore Users: Will experience downtime Tuesday, September 15 at 8pm for approximately 1 hour and then Tuesday, September 22nd 6am to 8am. No access is permitted during those times.
- McKesson Users: There will downtime Monday, September 28th from 9pm to 5am. No access is permitted at that time.
More to come on all the above. The IT department thanks you for your understanding while they do some maintenance to the system.

Encore Maintenance
The MIS Department is scheduled to perform routine maintenance on the Encore system Tuesday (September 15) beginning at 8pm. Maintenance is expected to take an hour, but an email will go out once complete.
Please plan accordingly and feel free to contact the MIS department with any questions or concerns.

Setting up a Conference Call
We are currently making a transition from AT Conference to Infinite for conference calling, saving the organization 33% per call. More information regarding this change and how to use the new numbers with be forthcoming. In the meantime, we are asking that when having conference calls of four or less parties that you use the conference feature of the phone system in your respective offices.
For Avaya:
Setting up a Conference Call
- From the Phone screen, select your active call.
- Press Conf.
- Dial the telephone number, or call the person from the contacts list, or the History list.
- Press Join when the person answers the call.
- To add another person, press Add and repeat Steps 3 to 4.
Note: If you have initiated the conference call and dropout, the conference is terminated.
For ShoreTel
Setting up a Conference Call
To conference a party into a connected call:
- Press the Conference button. The current call is put on hold.
- Dial the extension of the party you would like to conference.
- Press the Confrn soft key to ring the party directly.
Or
- Dial the number and press the Conslt soft key to speak with the recipient before ringing him or her into the conference call.
- After consultation, press the Yes soft key to complete the conference call, or press the Cancel soft key to return to the original two-party call.
- Step 4 Repeat to add additional parties.

Encore Documentation Reduction Committee: Change #3 - ROC-Referral/Orders Reviewed

When completing a Resumption of Care (ROC), it is no longer necessary to sign/initial the page one and turn it in to the CSA. Instead, document that the referral and orders were reviewed by checking off "ROC-Referral/orders reviewed" in the POC Communication to MD link in the Risk/ADS tab.
NOTE: Any paperwork not originally received from coordinator/CSA, etc. should continue to be sent to Medical Records to be scanned and added into the patient's clinical record. This includes medication lists from MD office, etc. Please contact your Patient Care Manager with any questions.

Q3 & Q4 OASIS Education Sessions
The following are the scheduled Q3 AND Q4 OASIS Education schedules. All clinical staff are mandated to attend 1 OASIS Education session per quarter, this includes RN (staff and contract), LPN, clinical managers , PT and PTA's, and clinical coordinators. Quarter 3 reviews Functional OA information and Quarter 4 will review miscellaneous OA education and COPS.
The QA Department has scheduled out many sessions in all offices for the final two quarters of the year to accommodate multiple offerings at multiple sites. Please work out the time that you choose to attend with your individual manager, you may attend at any site.
Staff must attend ONE education session per quarter. Our goal is to maintain our current best practice and strive for the top 20% in our reportable outcomes...remember that we are at 4 Stars for VNACN and 4.5 Stars for VNAB, with a goal of 5, we would like to keep the momentum going!!!
