Struggling With What to Wear?

Are you struggling on how to dress this summer? Has the updated dress code have you wondering what to wear? How can you maintain a professional image when you may be entering homes without air conditioning and temperatures reaching 100 degrees?
Well we have some great ideas that will keep you cool and stylish throughout the summer months! Whether you are visiting patients or working in the office every day the solutions are easy and affordable. Suggestions have come from staff who are veterans in working in these conditions. What could be better than scrubs and a light cotton shirt when you are out on the road? The pants are extremely light weight and durable. Knee length shorts with durable shoes are another alternative. Don't be tempted to wear a tank top and shorts. Sandals would keep your feet cool but will not keep you safe from unwanted critters who may want to come home with you.
Remember you are a professional and patients need to see you as such. Looking professional and organized sends a message to patients that you are knowledgeable and work in collaboration with their physician to provide care. This includes having all the supplies needed to complete the visit and communicating to the patient when you will arrive.
Look around at those who have provided services to you. How do they appear? What makes you value their service? Do they take the time to listen to your concerns? Do they have the tools needed for the job? Are they organized? Do they arrive at the stated time? What is your definition of professional? Share your thoughts with connecter@vnacarenetwork.org.

Celebrating Atrius Health
Executive Leadership at Atrius Health recently approved a plan to hold a "Celebrating Atrius Health" week to serve as celebration of the merger of the legacy groups (Dedham Medical, Granite Medical, and Harvard Vanguard Medical Associates), and in recognition of the hard work and dedication all staff have exhibited during the process.
Each Atrius Health and VNA site will hold a celebration that works for their unique location and staff schedules during the week of July 6th, the first full week post merge. A VNA Office Champion has been identified at each location and more details will be forthcoming in the next week as to the specifics of your office's celebration.
This is a great opportunity to not only celebrate the merger of the physician groups, but also the strong and collaborative relationship between the VNA and Atrius. Please stay tuned for more information.

Hospice Updates

|
Meet Deborah Turiano, MD, HMDC, Associate Medical Director! Although Dr. Turiano has been with VNA Hospice Care since 2011 as a part-time Associate Medical Director, she is now working full-time in our Woburn office taking care of the needs of not only Teams A & B in Woburn, but also the DeRham House.
A nationally certified hospice Medical Director, Dr. Turiano is one of the first groups of Hospice Medical Directors to earn this certification in the country. She has spent the last 30 years caring for critically ill patients and their families in both MA and NH. She attended the University of Buffalo School of Medicine and Biomedical Science, and pursued her post graduate education at Case Western Reserve University Hospitals and Harvard Medical School.

Dr. Turiano resides in Andover with her husband, Dr. Anthony Turiano, who has been a local practicing family practitioner for 27 years. They have 4 grown sons.Please join us in welcoming Dr. Turiano to our organization!
Amy Daley, RN, AD, has been promoted to the position of Clinical Manager of the Rose Monahan Hospice House. Amy has served at RMH as team leader for the past year. She brings her hospice house clinical manager experience from working at the Merrimack Valley Hospice House for the previous three years. Amy is a skilled clinician & has a good handle on the needs of our hospice inpatient facility.
Cyndi MacHenry, RN, AD, who has been a hospice nurse manager working out of the Woburn office for the past five years, will assume the responsibility of the day to day operations as clinical manager of the Tippett Hospice House in Needham & the deRham Hospice House in Cambridge. Cyndi will bring her years of hospice experience to further strengthen the good work being done at each house.
Mary Sheils, RN, BSN, has been promoted to the role of Clinical Director for VNA Hospice Care. In her role, she will be responsible for the agency's nurse manager team, the hospice psycho/social managers, the hospice quality team & the hospice education nurses. Mary joined the VNA Hospice Care team in 2013. She has over 37 years experience in nursing, focusing in the areas of hospice and palliative care, providing management, education and consulting skills.

Addressing the Crisis: Opioid Prescription Monitoring at Atrius Health
Opioid addiction in Massachusetts has become a serious problem with more than 1,000 residents dying of opioid overdoses in 2014. At the same time, pain management is an essential part of every medical practice, requiring clinicians and care teams to carefully consider the best treatment for patients who are suffering. Opioid medications can play an important role in managing pain, though misuse or abuse of opioids can also lead to significant patient and societal consequences.

Nepal Earthquake Relief Fund Update Thank you to all who contributed to the Nepal Earthquake Relief Fund. A check for $7,352.92 was sent to Direct Relief International, one of the largest nonprofit groups in the U.S. Direct Relief International will allocate 100 percent of funds raised to their relief efforts - like the ongoing emergency medical relief they are providing in Nepal - without deducting any overhead or administrative costs. The Nepal fund is now considered closed. We would also like to thank Harvard Vanguard's Watertown and Wellesley sites for making in-kind donations that will be put to great use in Nepal. Thank you to Linda Drake-Ferreira, Project Manager, and Kelli Murphy, Operations Manager, who helped coordinate Watertown OB/GYN's donation of a fetal monitor. Thank you as well to Linda Plukas, Process Improvement Leader, Kerri Bodenstein, Clinical Support Supervisor II and Maya Fox, Medical Assistant I, who helped coordinate Wellesley OB/GYN's donation of various medical supplies. Both donations were gifted to IMEC, an organization that reduces environmental waste by recovering surplus equipment and supplies and bringing these materials and hope to impoverished communities who need humanitarian support, such as the people of Nepal.

McKesson System Upgrade for VNAB Home Care Rescheduled

The McKesson upgrade has been rescheduled for the night of Wednesday, July 15th. Changes for clinical staff were communicated in the Connecter dated Tuesday, June 23rd. They will be re-posted in the Connecter the week of the upgrade. There will also be a small work-around posted for rehab service orders to overcome a problem that was detected. These are the details regarding the update:
The night of the update:
- The system will be shutdown at 9pm sharp on Wednesday, July 15th
- During the shutdown the following systems will be unavailable:
- Horizon Homecare for VNAB and Hospice
- TeleHealth Interface
- WebChart
- Web Referrals
- iPad Users and Office Staff:
- You must be signed out of the system by 8:45pm to avoid losing any documentation
- Please do not log on again before 6am the next morning
- You will be required to change your McKesson password when you log in.
- iPad Users and Office Staff:
- Full detail and instructions regarding the ESD transfer will be communicated in an email
- As you are already aware the ESD transfer will need to be performed in the office via network cable and will take about 30-45 minutes. Further detail to follow.

Two FDA Alerts
Zerbaxa (ceftolozane and tazobactam): Drug Safety Communication - FDA Cautions About Dose Confusion and Medication Errors
ISSUE: The U.S. Food and Drug Administration (FDA) is warning health care professionals about the risk for dosing errors with the antibacterial drug Zerbaxa (ceftolozane and tazobactam) due to confusion about the drug strength displayed on the vial and carton labeling. (See the Drug Safety Communication for Label Photos).
FDA evaluated seven reported cases of medication error that occurred during preparation of the dose in the pharmacy due to confusion with the display of the strength of individual ingredients on Zerbaxa's vial labels and carton labeling. Listing the individual drug strengths led to confusion because it was different from labeling for other drugs in the beta-lactam/beta-lactamase class that express strength as the sum of the two active ingredients. In some cases, this led to administration of 50% more drug than was prescribed. No adverse events were reported among these seven cases.
BACKGROUND: Zerbaxa is used to treat complicated infections in the urinary tract, or in combination with the antibacterial drug metronidazole to treat complicated infections in the abdomen. Antibacterial drugs work by killing or stopping the growth of bacteria that can cause illness.
RECOMMENDATION: FDA urges health care professionals and patients to report side effects and medication errors involving Zerbaxa to the FDA's MedWatch Safety Information and Adverse Event Reporting Program
Mucinex Fast-MAX Products: Recall - Incorrect Labeling
Including certain lots of Mucinex Fast-MAX Night Time Cold & Flu; Mucinex Fast-MAX Cold & Sinus; Mucinex Fast-MAX Severe Congestion & Cough and Mucinex Fast-MAX Cold, Flu & Sore Throat
ISSUE: RB (formerly Reckitt Benckiser) has recalled certain lots of liquid bottles of Mucinex Fast-MAX Night Time Cold & Flu; Mucinex Fast-MAX Cold & Sinus; Mucinex Fast-MAX Severe Congestion & Cough and Mucinex Fast-MAX Cold, Flu & Sore Throat because the over-the-counter medications, which correctly label the product on the front of the bottle and lists all active ingredients, may not have the correct corresponding drug facts label on the back. This mislabeling could cause the consumer to be unaware of side effects and/or risks associated with the ingestion of certain product ingredients which include acetaminophen, dextromethorphan, guaifenesin, phenylephrine and/or diphenhydramine.
Consumers could take a product with undeclared levels of acetaminophen, dextromethorphan, guaifenesin, phenylephrine and/or diphenhydramine. Consumers would not be adequately warned of side effects which could potentially lead to health complications requiring urgent medical intervention, particularly in the case of acetaminophen use in people with liver impairment.
RECOMMENDATION: RB is asking consumers to responsibly dispose of any unused product in accordance with the following recommended guidance for drug disposal in your household trash:
- Mix liquid medicines with an unpalatable substance such as kitty litter or used coffee grounds;
- Place the mixture in a container such as a sealed plastic bag; and
- Throw the container in your household trash

HHA Corner

This week I would like to bring the attention to shower chairs. When it is identified the patient needs a HHA, please check if the patient has a shower chair.
The HHA is only able to assist with showering if there is a shower chair in the home. If the patient has one, please note it in the DME section of the 485 and note it on the HHA assignment sheet. If the patient does not have one, please refer to OT.
The aide will provide a sponge bath until the chair is obtained. Thanks for your help with this!

HHA Corner

The HHA Department is responsible for filling the case as soon as the referral is received. We work with a number of contract agencies. Some areas have wait lists, other areas are filled very quickly.
I have been reviewing the cases that have not received an aide timely, and have identified a contributing factor on the clinical end. The process for HHA placement is completely dependent on the HHA department receiving a complete HHA Plan of Care.
When the HHA Plan of Care comes over incomplete, the HHA Supervisor, Amanda Burke, communicates to the team asking for completion. The turnaround time is ranging from the same day to up to 4 days. If the patient lives in one of the areas that services are limited, this situation could potentially add an additionally 4 days to the wait.
How can you help?
If you receive notification that the HHA Plan of Care is incomplete, please make it a priority to complete it as soon as possible. The HHA Assignment will reprint to the department ONLY when the assessment has been changed. Let us know when it is completed and we will get it printed and start the search! Otherwise it will contribute to a further delay.

There is no charge for this. While the wireless industry is constantly working to combat unsolicited text messages (SPAM) sent to consumers, you play an important role in the fight against spam by reporting it!

New Accounts Payable Email Address and Check Request Form
The accounts payable department has a new email address, AP@VNACareNetwork.org. If you need to submit an invoice, or have follow-up questions about an open invoice, you should use this new email address for both internal and external check requests.
If you possess an agency-issued credit card, we ask that you also submit those receipts to this email account. Cardholders are reminded to submit their monthly receipts in order to assure expenses are coded to the appropriate accounts.
Also, if you are working with a new vendor, please request a W9 form and send the completed document to the accounts payable department for filing. This is to ensure we have all vendors checked in the Office of Inspector General as well set up for proper exemption status.
All paper invoices should be mailed directly to the accounts payable department; 333 Turnpike Rd Southborough MA 01772. Having them mailed directly to the Southborough office ensures invoices get processed accurately and timely.

Below is the Quarter 2 OASIS education schedule for all offices. Our topic this quarter is Wounds. Mandatory attendance is expected for all clinical professionals. You may attend any site but be sure to sign in on the attendance so you will get credit for attendance. If you do not attend any of the sessions, you will need to complete the makeup test and have a meeting with the PCM in your district to be sure you are understand the Q2 content.
