Beginning on April 3, 2015, all Ages & Stages Questionnaires (ASQ) will be recorded online only. The Early Learning Coalition will no longer be using Scantrons to record ASQ answers.

New Process

On or around the first of each month, you will receive a packet that includes a "Face Sheet" for each child due to be screened. This packet will be mailed to you unless you participate in the ShareFile system, in that case, the packet will be uploaded to your ShareFile folder labeled ASQ's.

You will then access the ASQ online to record the answers at: https://www.asqonline.com/family/e5b854 

This link is also posted on the Coalition's website homepage on the "I am a Provider, I want to..." menu, select Complete ASQ Online and click on "Go".

Detailed instructions will be provided in each screening notification.  You can also access more information on this process on our website at  http://www.elcpinellas.net/screening.php.

If the child is already in process of being evaluated (i.e.- FDLRS, Early Steps) or is currently receiving services (i.e.- Speech Therapy) please write that in the comment section of the ASQ.

If the child is no longer in your care, please fill in the bubble indicating that on the Face Sheet and mail or fax to 727-548-1509 to Early Learning Coalition.
