TOP TIP FOR ACCREDITATION
Health Summaries
At least 75% of active records should include an up-to-date health summary and 90% of these records should include a record of known allergies - GPs need to be routinely recording known allergies in their patient's health records.
A satisfactory health summary includes:
- adverse drug reactions
- current medicines list
- current health problems
- relevant past health history
- health risk factors e.g. smoking, alcohol
- immunisations
- relevant family history
- relevant social history including cultural background
Our practice incorporates health summaries into active patient health records
Note: An "active" patient health record is defined in the Standards as the record of a patient who has attended the practice three or more times in the past two years.
