Every summer the best and brightest health care minds in Colorado come together in Keystone to think and learn at the Colorado Health Symposium. We've piggy-backed on this great tradition by starting a yearly ritual of our own.

On Tuesday, members of the Colorado Network of Health Alliances met in Keystone for our 2nd Annual Statewide Meeting. At the meeting, alliance leaders from across the state put their heads together to formulate a strategic vision and collective action for the year to come.

At the end of the day the energy in the room was palpable. We're thrilled to work beside these great local leaders and can't wait for the collective work that awaits.

Aurora Health Access
In 2009, Together Colorado and MCPN held a community meeting to start a conversation about health.From that meeting, Aurora Health Access (AHA) was born in January 2010. AHA has been meeting ever since as a community collaboration.

 

Aurora Health Access works to...
* Improve access to care in Aurora. The group's actions work towards increasing health insurance coverage, care coordination and access to specialty care.

*Increase general community education, networking and mobilization. The group does general community outreach and education, holds community wide quarterly meetings and maintains a newsletter.

NEW ALLIANCE: Welcome to Longmont Community Health Network
The Longmont Community Health Network formed in early 2012, catalyzed by the Public Safety Department in collaboration with local health care leaders. The group began by targeting the high utilizers of emergency services and has since expanded bringing it's collaborative effort to other areas of need within the area.

ANNOUNCEMENT: Assistance Network Launch
Connect for Health Colorado is developing the Connect for Health Assistance Network. Assistance Sites across Colorado will help deliver local, in-person education and help individuals and small employers apply for health insurance coverage. Colorado Network of Health Alliance Members North Colorado Health Alliance, Northwest Colorado Community Health Partnership, and Tri-County Health Network all received training grants to become Assistance Sites. Congratulations!
RESOURCE: Healthy Transitions Colorado Kick-Off and Oppurtunities
Healthy Transitions Colorado launched their campaign in early July. The state-wide collaboration is uniting efforts to improve care transitions and reduce readmission rates. Learn about how they are engaging everyone from hospitals, to food banks, to transportation companies in their efforts.
