Please return completed forms to Yvonne Curtis in care of the Synod Office, fax to 267.323.3777, or email to ycurtis@sepa.org

Please Help Us Save on Health Costs

As of Feb. 1, 25% of our eligible members had completed the assessment. Please take time to take the assessment by the end of February (and your spouse, if covered by the plan).

  • You'll get a $150 credit to cover out-of-pocket costs.
