Group Health Cooperative of South Central Wisconsin's (GHC-SCW) commitment to quality is ingrained in everything we do. Delivering on our promise is centered around our focus on the Triple Aim. We believe part of achieving the ultimate patient experience, as well as improving the health of our population at the lowest cost, includes providing care through integrated care teams.
In 2014, we incorporated Behavioral Health Providers into our Primary Care teams. Our Providers recognized the innate connection between behavioral/mental health issues with physical health. The result of our efforts has been remarkable. This enhanced model and approach led us to take the next step toward innovation in patient care. We have now integrated our Clinical Pharmacists into our care teams to assist our patients when they need us. These Pharmacists help counsel those who are on many medications, in an effort to increase adherence to compliance. Additionally, they assist in recommendations to alternative pharmacological interventions that can improve our patient outcomes. Our integrated care teams truly place our patients at the center of all we do, and it is working!
Through partnership and commitment to patient care, we value the work that all our Providers do throughout the communities we serve. I look forward to working with you in the upcoming year to continue to provide exceptional service to our members!

cooperative news | | NCQA Rankings to Change to Star Ratings The National Committee for Quality Assurance (NCQA) works as an independent, non-profit entity to improve the health care system. Each year, NCQA uses the Healthcare Effectiveness Data and Information Set (HEDIS�) as a measure of clinical quality, the Consumer Assessment of Healthcare Providers and Systems (CAHPS�) as a measure of patient experience, and various standards from the NCQA Accreditation process to report health plan performance in numeric rankings. 2015 (Measurement Year 2014) Results
Beginning with the 2015 results, a new NCQA star rating system will replace the numeric rankings. Star ratings will be calculated on a scale of one to five measured to the nearest half point. A five star rating will be the highest score. Performance ratings will include three subcategories also scored on a scale of one to five: consumer satisfaction, prevention and treatment. The overall rating will be based on the weighted average of a plan's HEDIS� and CAHPS� scores, plus accreditation standards. Accreditation standards will be 10% of the weight of the valid HEDIS� and CAHPS� measures that a plan submits.

Digital GHC-SCW Member ID Cards GHC-SCW members can now download and print a digital copy of their member ID card through GHCMyChartSM, and save their digital card to a computer or mobile device. GHC-SCW digital member ID cards are identical to the printed cards and can be used to check in at appointments or to access services. Members can follow these simple instructions to view and download a digital version of their GHC-SCW ID card.
- Visit www.ghcscw.com and log in to GHCMyChartSM.
- Hover over the "My Record" tab and select "My ID Card" under the "My Insurance" section.

Dear Colleagues,
The GHC-SCW Quality Improvement (QI) program is dedicated to improving the quality of clinical care, service and safety provided to all of our members. Each year, we review our QI program efforts, prepare our annual report of the previous year and develop a work plan for the year ahead. The work plan includes information about our processes, goals and project outcomes as they relate to member care and services.
As we evaluate the QI program, we refine goals and priorities based on how performance measures compare against national benchmarks, best practices, internal benchmarks and our members' and Providers' needs. Improvement efforts primarily focus on clinical quality, behavioral health, service quality, patient safety and member experience.
I invite you to view our 2014 QI Annual Report and see the work going into furthering our continuous improvement journey. We remain committed to quality improvements that prioritize Triple Aim - "Better Care, Better Health and Lower Costs" - and focus our efforts in 2015 on patient experience and diabetes care improvements.

important member information | | Disease Management Programs Help Members Manage Chronic Conditions
If you are looking for assistance helping your patients manage chronic conditions, you can refer members to GHC-SCW Disease Management. Members enrolled in Disease Management programs are informed how to monitor and manage their condition, receive periodic educational materials and self-management tools, as well as care reminders by phone, mail or within GHCMyChartSM. Classes, information about events and individual consultations with specially-trained health educators are also available.
Programs Disease Management program descriptions include instructions on how to use Disease Management services and how the program works with your patients. - Living Well with Diabetes helps patients learn how to better understand their condition, self-manage diabetes through education, nutrition, physical activity, medication regimens and appointments with Dietitians and Certified Diabetes Educators.
- Asthma Disease Management works with members' Primary Care Providers and the Asthma Educator to provide the tools and knowledge needed to take control of asthma.
- Heart and Vascular Disease Management assists members' Primary Care Providers in educating patients on ways to control cholesterol, blood pressure and other risk factors associated with heart and vascular disease.

Access to Care Management Staff
Our Care Management team is accessible to members, their caregivers and Providers to discuss issues related to case management and utilization management. Case management may consist of a team of nurses, social workers and additional support staff who will assist in educating and navigating members through the delivery of health care. The Utilization Management team reviews the requests for services, diagnostic tests, procedures, etc. that may require medical necessity and/or appropriateness decisions by using evidence-based guidelines to support determinations. You can reach the team by phone. Fax and confidential voicemail are available after-hours and on holidays.

Care Management Functions
If a member is facing multiple chronic diseases such as, but not limited to, diabetes, congestive heart failure, chronic obstructive pulmonary disease, cancer or depression/AODA, the GHC-SCW Care Management team may be able to help. The Care Management staff performs two distinct functions: utilization management and complex case management. The Case Manager provided through the Complex Case Management program helps members with complex medical situations navigate the health care system, identify barriers to achieving goals and achieve goals that improve their quality of life. The GHC-SCW Utilization Management staff members perform prior authorizations/referral reviews for medical necessity of services being requested prior to services being rendered.

Complex Case Management
GHC-SCW Complex Case Management is available to members with complex medical conditions or for whom a critical event has triggered a need for rehabilitation or complicated health care support. The goal of the Complex Case Management program is to assist members who have multiple or complex conditions in obtaining access to quality health care and appropriate services. Complex Case Management is intended to help members and their caregivers coordinate their health care needs and navigate the health care system.
Qualifications
For a GHC-SCW member to qualify for assistance from the Complex Case Management program, they must meet the following criteria:
- Valid GHC-SCW health care coverage for their medical/behavioral health services; and
- A chronic disease diagnosis of two to three co-morbidities (CHF, COPD, diabetes, CA, depression/AODA) and/or multiple hospital admissions and/or emergency room visits within six months; or
- A catastrophic event and/or the complexity of services
Evaluation
Evaluation for enrollment in the Complex Case Management program can be obtained through a member's Primary Care Provider, their Specialist or by the member or their caregiver directly calling the GHC-SCW Care Management department at (608) 257-5294. Members or their caregivers may also contact GHC-SCW by calling GHC NurseConnect at (608) 661-7350 (local) or (855) 661-7350 (toll-free).

Utilization Management
GHC-SCW Utilization Management staff is available for you to discuss issues or questions you may have related to the need for a referral/ prior authorization for a particular service, or to discuss a denied decision. Please contact the Care Management department at (608) 257-5294 or (800) 605-4327, ext. 4514. Care Management staff is available Monday - Friday, 8 a.m. to 5 p.m. If you have an urgent need outside normal business hours, please leave a voice mail and your call will be returned the following business day.

Member Rights and Responsibilities
Our mission is to provide high-quality, comprehensive and accessible health care services to members in an efficient and personalized way. To ensure that these goals are met, it is important that members understand their rights and responsibilities.
Rights
The right to:
- Receive information about GHC-SCW, its services and its providers, including the right to receive a copy of the GHC-SCW Patient Rights and Responsibilities.
- Be treated with dignity and respect in a confidential manner.
- Participate with your providers in making decisions about your health care.
- Participate in a candid discussion of appropriate or medically necessary treatment options for your conditions, regardless of cost or benefit coverage.
- Voice complaints about the service and care you receive without penalty or disenrollment.
- Receive notification and a rationale when case management services are changed or no longer needed.
- Receive a certificate outlining the coverage to which you and/or your family members are entitled, and to whom the benefits are paid.
- Ask questions regarding your medical plan coverage, the preauthorization process or claims payment.
- Submit complaints about appeals about GHC-SCW or the care we provide.
- Select a Primary Care Provider and to request a new provider without indicating a reason.
- Receive a full explanation of any charges billed to you as a result of care.
- Participate in the governance of GHC-SCW. Each member must be at least 18 years of age to be a voting member of the Cooperative and is encouraged to actively participate in its operation.
- Make recommendations regarding the organization's member rights and responsibilities.
- Receive informed consent, as required by law, prior to procedures or treatments. To the extent permitted by law, it is your right to refuse the recommended treatment and be informed of the consequences of this decision.
- Receive confidential treatment of all communications and records concerning your care, except as otherwise provided by law.
- View and receive a copy of your health records and x-rays upon receipt of written authorization.
- Receive a copy of the GHC-SCW Notice of Privacy Practices.
Responsibilities
The responsibility to:
- Be considerate of others.
- Observe safety and smoking regulations in all GHC-SCW facilities.
- Treat GHC-SCW employees with consideration and respect.
- Provide accurate and complete health care information.
- Use facilities and equipment properly.
- Read and understand your coverage.
- Be on time for appointments and inform the clinic in advance when appointments cannot be kept.
- Follow plans and instructions for care as agreed to with your provider.
- Understand your health problems and participate in developing mutually-agreed-upon treatment goals.
- Pay your financial obligations under the benefit plan.
- Know and confirm your benefits before receiving treatment.
- Obtain preauthorization for services indicated in your certificate.
- Notify GHC-SCW of changes in your address, phone number or family status.

The GHC-SCW Care Management department uses the Milliman Care Guidelines (MCG), which are evidence-based guidelines, along with other utilization management criteria to ensure the most appropriate course of treatment for our members, while providing a clear and consistent platform for care decisions. These guidelines enable GHC-SCW to improve operational efficiencies by providing clinical indicators of when it is appropriate to transition a patient to the next level of care.

GHC -SCW Commercial HMO Member Prescription Benefit
General updates and formulary changes are communicated semi-annually in Practitioner Update publications. Formulary updates that result in significant restrictions or changes are communicated directly to affected members and their Practitioners when they occur.
Generic substitution is encouraged and allows the lowest copayment available for a benefit plan. Drugs with criteria such as quantity limits (QL), step therapy (ST) and prior authorization (PA) are shown on the formulary. Therapeutic Interchange is not a health plan program.
