New Additions to the Tri-Town Health Center

Tri-Town welcomes the following additions to better the community

  • To assist the general public, as well as clients, about the upcoming Affordable Care Act, Tri-Town has hired Jim Carleton. Flyers will be distributed to the public with Jim's contact information if there are any questions.

 

  • In an attempt to respond to patient needs,Tri-Town has developed a call center. The call center handles incoming calls from Health Center patients and handles all patient requests. The intention of the development of the call center is allow patients to get responses to their requests more rapidly, to be able to get health care advice from their provider in a more timely manner, and to be able to schedule appointments in a more responsive and timely way.  
  • Tri-Town has added John Roies, a full time Nurse Case Manager to the Care Team to meet with patients to help provide:
  1. Care Coordination Services - Helps patient coordinate the care they may be receiving at multiple specialists. The goal is to help patient's access all of the specialty care they may need in a way that is understandable and fits with their busy lives.
  2. Disease Education - Meets with patients who are dealing with chronic diseases (e.g. Diabetes, Asthma, Coronary Artery Disease) and helps them to better understand their condition. Also helps patients to understand what patients can do themselves to address their conditions.
  3. Self Management Support - Meets with patients to help them develop self management goals that will lead to improved help. Self management goals might include things like exercising more, changing your diet, or learning techniques to manage stress.
  • Dr. Erica Waddington is our new Post-Doctoral Fellow from Brown University. She will be with Tri-Town for one year and will be completing a post doctoral fellowship in Child and Maternal Health. She will be providing family practice, gynecological, and obstetrical care for patients of the health center. Dr. Waddington will be working with Suzanne Lowe, CNM, in partnership with Memorial Hospital to offer patients of the health center to have their deliveries at Memorial Hospital.
      
  • Tri-Town has hired Nicole Deschenes as a Family Nurse Practitioner in the health center. Nicole joined Tri-Town full time as a family nurse practitioner in September of 2013. Nicole graduated from the University of Massachusetts and has extensive experience in Community Health Centers. 
 

Tri-Town Attends the 2013 Apple Festival  

The 26th Annual Apple Festival took place in Johnston

Tri-Town Community Action Agency attended the 25th annual Apple Fest on Saturday, September 29, and Sunday September 30, from 10am - 6pm at the Johnston Memorial Park. In addition to giving general information about services from the agency, Jim Carleton was there talking about the new Health Care Law.

 

Tri-Town Attends Rally 4 Recovery

Tri-Town Community Action Agency participated in the Rally 4 Recovery on September 21, 2013. "The Rally 4 Recovery is an annual festival organized by a coalition consisting of persons in recovery from addiction to alcohol and other drugs, their family members, friends and allies. It is a community building effort, seeking to strengthen the bonds among people in recovery, their family members, friends and allies and increase the number of supports available to them in the community. The Rally also serves to foster a culture of recovery in Rhode Island, supporting the belief that everyone has a right to, and is capable of, recovery of addiction to alcohol and other drugs." LIHEAP Guidelines Released for 2014   

Income maximum for families slightly lowered from 2013

Tri-Town Community Action Agency is seeking applications from individuals and families in need of help with their fuel bills. Tri-Town releases heating assistance grants to homeowners and renters alike based on income eligibility in the ares of areas of North Providence, Johnston, Smithfield, Glocester, and Burrillville. The department is located at 33 Maple Avenue in North Providence. Please call 401-351-2750 extension 1913 for more information regarding some assistance with the rising costs of energy.
