A Plan for our Future 

Like hospitals and clinics around the country, Contra Costa Regional Medical Center & Health Centers is adapting to today's rapidly evolving health care environment. To meet these new challenges, it is essential that we plan for the future with a shared vision and clear priorities.

 

That's why I am excited about the recent launch of our 2015-2020 Strategic Plan. This ambitious 5-year plan will not only define who we are, but who we want to be.

 

The plan recognizes and builds on the amazing work you all do every day and serves as a roadmap to focus our efforts on our three priorities: being patient and family centered; continuous improvement; and delivering value. 

 

It will guide us through the next five years as we move toward our goal of making our community the healthiest in the nation. It's a bold goal worthy of all of you and those we serve. It won't happen overnight, but we will get there. We are already shedding the outdated and outmoded image of ourselves as a place of last resort. 

 

We all have a role in ensuring the success of our plan, which is the product of intense effort and input from staff across our delivery system, from the frontline to the executive team, as well as from our community and patient and family member partners.

This first year is an important period during which we will be gathering information and developing initiatives to support our priorities. Over the next few months, you will be hearing more details from your supervisor and through this newsletter and iSITE. 

 

Please join our efforts to create optimal health for all through respectful relationships and the delivery of high quality services. I'm excited to work with all of you to make Contra Costa County the healthiest community in the nation.

Improvement Opportunities in Physician Leadership Roles and Structure
It has been nearly three months since I transitioned into my new role as chief medical officer. What is clear to me, even more than before, is that we have dedicated,
motivated, and ethical staff who inspire trust in our system.

I wanted to share with you some of my goals to help support the great improvement work already happening at CCRMC & Health Centers. These goals focus on:

1. Patient access
2. Provider efficiency and satisfaction
3. Surpassing community standards in quality of care
4. Networking our services with other systems
5. Physician administrative structure improvement

These goals remain aligned with the aim of the CCRMC & Health Centers strategic plan. In regard to the last priority, the changing environment for our clinical services require physician leadership be efficiently organized and appropriately supported to allow continuous improvement in care delivery. This is why I am excited that our executive leadership team authorized a project to assess improvement opportunities in physician leadership roles and structure. 

To help us in this task, we have partnered with Huron Healthcare. Currently we are in the midst of physician governance and leadership redesign planning sessions, which have been undertaken to illustrate and redefine ideal state physician leadership roles and responsibilities. Although this will be iterative and measured in development, I hope to clarify, simplify, organize and support physician operational roles at the 
completion of this process.

Scenes From the July Disaster Drill at CCRMC

On July 16, we tested our ability to respond to sudden medical surge following a large-scale earthquake. Disaster equipment was set up to accommodate rapid movement of patients during a sudden medical surge.  As mock patients arrived to the hospital, they were led through registration, triage and treatment. Health Leads  Execs and Board Members Visit West County Health Center 
Clinic Services Manager Caroline Killough (far left) gives Health Leads board members a tour of the West County Health Center. 
Earlier this month, representatives from Health Leads USA visited West County Health Center (WCHC) to get a firsthand look at our successful year-long partnership with the nonprofit agency to addresss addressing underlying socioeconomic issues that affect health.

With the help of Health Leads, providers at WCHC connect patients with needed non-medical services, including food assistance, job training and child care and afterschool programs.

Rebecca Onie, Health Leads' CEO and co-founder, described WCHC as an "exquisite example" of implementing the Health Leads service model. 

CCRMC & Health Centers' New Integration Program Chief 
Victor Montoya

Victor Montoya, the longtime head of Contra Costa County's Adult/Older Mental Health Program Chief, is joining the CCRMC and HCs' team as our Integration Program Chief.


 
In this role, Victor will help lead integration of behavioral health and primary care in our health care delivery system. He will be reporting to Chief Quality Officer Shelly Whalon and working in partnership with our Medical Director of Process Redesign and System Integration, Dr. Chris Farnitano. Victor will also help support acute and emergency operations of behavioral health services at CCRMC, including our Psychiatric Emergency Service. He will continue to oversee the county Office of Conservatorship as well as liaison with the state hospital system to support seamless transitions for those in need of these specialized services.
