Building a Provider Resource Directory

We are excited to start building our national Provider Resource Directory in partnership with our parent foundation, the Foundation for Excellence in Mental Health Care.

The Foundation is now accepting applications from providers who are committed to providing empowering, holistic and recovery-focused mental health care.

Providers: If you would like to be listed in our national directory, please click the link below to download an application.

Individuals and Families: You can download our application and share it with  current providers or use it to interview new providers. This is a great way to start a recovery dialogue in your community!
