AHRC New York City is partnering with 
the Council on Quality and Leadership, (CQL), 
to assist us in better serving our community.
.
CQL has worked with agencies for decades, focusing on ensuring excellence in agencies that support people with disabilities and other vulnerable populations.
.
In establishing this relationship, we will conduct a number of conversations with the people we support.
.
This initiative will allow us to discover what really matters to those who we support, and how they define quality services. In our conversations we are able to identify dreams, goals, and wishes from the individual's perspective.
.
These conversations will occur 1-2 months prior to the person's Annual Individualized Service Plan meetings.
.
The information collected will be shared with all service providers, so that the person's desires may be integrated into his/her daily planning. This will allow each person to set the agenda for their meetings.
.
CQL Conversations
.
CQL conversations help shift the definition of quality, from program
focused to individual focused. To begin the conversation, we first meet with people we support and discus topics such as health and safety, respect, friends, where they live and work. Each CQL conversation lasts from 1 to 2 hours.
.
Each person is provided the opportunity to define the outcomes that are important to them.  We use Personal Outcome Measures to determine what factors are most important in the lives of those who we support. These factors are separated into three parts for each individual:
.
Myself: Who I am as a result of my unique heredity, life experiences, and decisions.
.
My World: Where I work, live, socialize, belong, or connect. 
.
My Dreams: How I want my life (self and world) to be.
.
After a conversation is completed, we share the collected information with service providers to incorporate the individual's priorities into
planning. We want to provide the proper services and supports, for a higher quality of life. 
.
Encouraging Dreams 
The conversations celebrate and recognize that each persons story is different. We are encouraging people to use their voices and to realize that they have choices.
. 
Through the encouragement of dreams, AHRC New York City provides a strong sense of empowerment to the people who we support.  
. 
The more we, as an organization, and families are aware of their priorities, the more we can help individuals to reach their goals.

Each person has a dream, and AHRC New York City is committed to supporting  
people as they work toward their goals, each and every day.

Enhancing Quality
.
AHRC New York City has been provided with a unique opportunity to improve how we deliver quality services and supports to people. 
.
With the help of all employees, our Quality Improvement Department is organizing person-directed supports, to ensure that we focus on what really matters to people in our community.
.
We assist all individuals in realizing their goals and dreams, with close attention to what they consider to be a quality life. This is a new approach to promoting person-directed planning and supports.
.
These conversations will continue throughout the years and across the organization. A number of AHRC New York City staff are involved in these conversations and are working toward building a quality
environment for each person.
