Voucher Endorsement Form

Dear Parents and Guardians,
As a Scecina Memorial family that is enrolled in the Indiana Choice Scholarship (voucher) Program, the State of Indiana requires that you sign two Voucher Endorsement Forms before receiving the full value of your voucher. The first Endorsement Form is due in September and the second is due in February.
The Voucher Endorsement Form is customized for each student and now available in the Scecina Memorial Tuition Office. Failure to sign this form by the deadline will significantly increase your tuition payments.
The Scecina Memorial Tuition Office has designated Tuesday, Wednesday, and Thursday, September 22, 23, and 24, as the signing dates. Please visit the Tuition Office in St. Francis Hall between 7 a.m. and 6 p.m. on these dates to sign your endorsement form.
