State Rep visits Falcon Park to see popular
D15 summer nutrition program in action 
   
State Rep. Elaine Nekritz joined District 15's School Nutrition Services staff in serving breakfast Monday morning to participants in Palatine Park District's summer camps at Falcon Park Recreation Center.

"I appreciated the opportunity to see firsthand how efficiently and effectively the Summer Nutrition Program works," said Rep. Nekritz. "The need for healthy meals doesn't end with the regular school year, and I'm grateful that the District 15 School Nutrition Services provides this valuable service."

Around 80 campers, counselors, and other youth from the community enjoyed the simple meal, which included packages of whole-grain cereal and graham crackers, fresh fruits, low fat milk, and 100 percent fruit juice. The breakfasts, which are served every day of the camp, are part of the District's Summer Nutrition Program, which has been offered at nine different locations this summer.

By the time the District's Summer Nutrition Program wraps up later this month, it will have served more than 11,500 free breakfasts to area youth age 18 and younger.

When District 15 launched the federally funded program seven years ago, that number only stood at about 6,700.

"The growth of this program is important because many students simply don't have access to nutritious meals during the summer break," said Kristin Voigts, School Nutrition Services Assistant Director.

District 15's School Nutrition Services offers these free breakfasts to the community throughout the summer through the U.S. Department of Agriculture's Summer Food Service Program. This program requires the District to offer at least one free meal per day in any of its schools with free and reduced-price lunch participation of 50 percent or more that are holding summer programs.

"Lack of nutrition during the summer months is a serious concern, and these meals meet nutrition standards set forth by the USDA," noted Mrs. Voigts.

Health exams required for some students            
For the purpose of safeguarding the health of children, District 15 enforces Illinois health requirements. These requirements consist of health, dental, and eye exams, and immunizations specific to grade and age.

Health exams are required for students entering preschool, kindergarten, sixth grade, and those who are new to the school. The Certificate of Child Health Examination form is available here (English PDF/Spanish PDF) and in the school office.

The exam form, completed and signed by a licensed healthcare provider, must be dated within one year prior to the first day of school and must include a diabetes screening and lead screening for children who are 6 years old or younger. A tuberculosis (TB) skin test is recommended.

The Health History section of the form must be completed and signed by the parent/guardian.

On October 15, students who have not provided proof of the required immunizations and/or health exams will be excluded from school according to District policy and Illinois School Code.  

physicals art Interscholastic Sports Participation
Annual health examinations are also required for interscholastic sports participation. Physical exams are required for junior high athletes before they may try out for an interscholastic sport. The exam form, completed and signed by a doctor, must be dated within one year prior to the start of practice or tryouts, and must be filed in the school nurse's office before the student can try out. Physical forms (Certificate of Child Health Examination forms) are available in the school office and online (English PDF/Spanish PDF).

Dental Exams
Dental exams are required for students in kindergarten, second grade, and sixth grade. The dental examination must have taken place within 18 months prior to May 15 of the school year. A waiver form is available in the case of an undue burden or lack of access to a dentist. The IDPH Dental Examination form is available here (English PDF/Spanish PDF) and in the school office. If a student fails to present proof of a dental exam or waiver form by May 15, the school may withhold the student's report card until the student presents proof: (1) of a completed dental exam, or (2) that a dental exam will take place within 60 days after May 15.

Eye Exams
Eye exams are required for students entering kindergarten and entering Illinois schools for the first time. A waiver form is available in the case of an undue burden or lack of access to an optometrist or to a physician who performs eye examinations. The State of Illinois Eye Examination Report is available here and in the school office. If a student fails to present proof of an eye exam or waiver form by October 15, the school may withhold the student's report card until the student presents proof: (1) of a completed eye exam, or (2) that an eye exam will take place within 60 days after October 15.
