back-to-school mailings
Your child's class assignment or schedule and a variety of other important back-to-school information such as school rules and procedures, District and school calendars, principal letters, and PTA information will be mailed to you in early August. If you do not receive this packet by Monday, August 17, please contact your school. District 15 students are entitled to bus transportation at no charge if they are residents of the District and live more than 1˝ miles from the District-assigned school they will attend, or in an area declared to be hazardous as determined by the Illinois Department of Transportation.
All bus assignments, as well as routes and stops, are determined by the Transportation Department, which will send in mid August letters detailing this information to parents whose children ride school buses. This information includes the student's morning and afternoon route numbers, the student's bus stop location, and the approximate times that the bus will pick up and drop off the student. Parents are asked to be sure the information on the bus assignment is accurate. If the information is not correct, or if you have not received it by Monday, August 17, please contact your school or the Transportation Department at 847-963-3900. Student fee statements
District 15 charges a book rental, material, and technology fee to help defray costs. Book/material/technology fees for the 2015-16 school year are: - Kindergarten -- $60
- Grades 1-6 -- $120
- Grades 7-8 -- $135
Other student fees are optional. They include interscholastic sports ($85/sport/year or $175 maximum/student/year, nonrefundable) and/or paid transportation fees for students living less than 1˝ miles from school. Student fee statements will be mailed to families at the end of August, and must be paid by November 1, 2015. A late fee of $50 per student will be charged for all student fees paid after November 1. Fees may be paid by cash, check, or credit card. Cash payments should be made by the parent in the office of the child's attendance school, where a payment receipt will be issued. Check payments are processed through the check processing center of the District's bank and should be mailed to Community Consolidated School District 15, 36246 Treasury Center, Chicago, IL 60694-6200. Credit card payments may be submitted online by visiting http://ccsd15.revtrak.net.

Health exams required for some students
For the purpose of safeguarding the health of children, District 15 enforces Illinois health requirements. These requirements consist of health, dental, and eye exams, and immunizations specific to grade and age.
Health exams are required for students entering preschool, kindergarten, and sixth grade, and those who are new to the school. The Certificate of Child Health Examination form is available here (English PDF / Spanish PDF) and in the school office.
The exam form, completed and signed by a licensed healthcare provider, must be dated within one year prior to the first day of school and must include a diabetes screening and lead screening for children who are 6 years old or younger.A tuberculosis (TB) skin test is recommended. The Health History section of the form must be completed and signed by the parent/guardian.
Required school immunizations are:
- Pneumococcal (PCV) (preschoolers only): children entering preschool shall show proof of having received vaccine, per schedule
- Diphtheria, tetanus and pertussis (DPT): children entering preschool shall show proof of having received four (4) doses of vaccine; children entering Grades K-12 shall show proof of having received three (3) doses of vaccine
- Diphtheria, tetanus, and pertussis booster adolescent (Tdap): children entering Grades 6-12 shall show proof of having received one (1) booster dose of vaccine
- Polio (IPV): children entering school at any grade level (pre-K-12) shall show proof of having received three (3) or more doses of vaccine
- Measles, mumps and rubella (MMR): children entering school at any grade level (pre-K-12) shall show proof of having received two (2) doses of vaccine
- Haemophilus influenzae (Hib): children entering preschool shall show proof of having received vaccine, per schedule
- Chicken pox/Varicella (Varivax): children entering Grade K, 1, 6, 7, or 9 for the first time shall show proof of having received two (2) doses of vaccine
- Hepatitis (HBV): children entering preschool and children in Grades 6-12 shall show proof of having received three (3) doses of vaccine
- Meningococcal (MCV4): children entering Grade 6 or Grade 12 for the first time shall show proof of having received one dose (1) of vaccine
Students who do not provide the required immunization and/or exam will be excluded from school according to District policy and Illinois School Code. On October 15, students who have not provided a health exam or required immunizations will be excluded from school. Students new to Illinois schools, who register midterm, have 30 days following registration to comply with the health requirements.
Physicals required before athletes can try out for sports
In addition, annual health examinations are required for interscholastic sports participation. Junior high athletes need physicals before they can try out for an interscholastic sport. The exam form, completed and signed by a doctor, must be dated within one year prior to the start of practice or tryouts, and must be filed in the school nurse's office before the student can try out. Here Comes the Bus GPS program now available for District 15 parents
District 15 now offers the Here Comes the Bus GPS program, which provides parents with online access (via computer or smartphone) to a wide range of information regarding the transportation services that the District provides their children.
The free program shows parents a map displaying a real-time view of the location of their child's bus in relation to their specific bus stop and school. It also shows parents their child's scheduled and actual pick-up and drop-off times for the current time of day. Using these features, parents can see the direction and pace the bus is traveling, and anticipate potential delays in pick-up or drop-off times.
Parents can also pay a fee and receive e-mail alerts and push notifications regarding the status of their child's bus.

Winston Campus Junior High teachers receive eight Richard Bokor Memorial Grants
Fourteen Winston Campus Junior High teachers have combined to receive eight grants totaling of $7,000 in memory of Richard Bokor, the late teacher, coach, and District 15 Board member who passed away in January.
The Richard Bokor Memorial Grants have been established to support English Language Learners (ELL) and to promote social studies. These were both passionate interests of Mr. Bokor, who began his 40-plus year career as an educator by serving as a social studies teacher at Winston Campus Junior High. The hope is that these grants will help the school's staff inspire and motivate their students to continue Mr. Bokor's great legacy.
The first recipients of the Richard Bokor Memorial Grants are as follows:
