Dear ASHI Annual Meeting Attendees,

The meeting evaluation site is now up and running! Please accept our apologies for the delay.

Click here to complete your evaluation. Your username is the email address that you registered with and the password is ASHI. 

The online certificate site will only be available through November 27, 2015.

After that date, the site will be removed and certificates will no longer be available. If you need a CEC certificate, you must complete the evaluation and certificate process prior to that date.

This year's evaluation is divided into sections (demographics, session evaluations, and overall evaluation). 

Your CEC hours will be automatically calculated based on the events you attended and evaluated. Upon completion of your evaluation, you may print your certificate immediately or have it emailed to you. If you choose to have the certificates sent to you via email, we strongly encourage you to save the email for future reference.
