Registration is now open for the ASHI Annual Meeting. 

THREE WAYS TO REGISTER
1. Online
Register online, payment must be made by credit card.

2. Fax
Complete the registration form in its entirety, including payment information, and fax it to the ASHI headquarters office at

3. Mail
Complete the registration form including credit card information or a check* made payable to:
