Transformations Loyalty Rewards
Program for 2014
 



 

Christie ClinicTransformations will begin a loyalty program in 2014 where you can earn points to receive free gifts and free food!  You can earn points while in all phases of the program.

We created this program to outline the "ideal" dieter behaviors.
They are the keys to success which will help you to get the weight off and keep it off. 
After all, that is the ultimate goal!

We believe one of the keys to success is attendance at your weekly visit while in phase 1. This will help you follow the program which will give you the results you desire.  We also know coming in for monthly and eventually yearly visits
in phase 4 are very important as well. 

We wanted to keep the program simple, so we will be utilizing a punch card system.  We hope that it will incentivize you to execute the keys to success.  

You will receive your loyalty card free the next time you are in!
You will receive points for the following:

5 points - Weekly visit and appropriate amount of food purchased.
10 points - Phase 4 maintenance monthly visit attendance.
10 points - Refer a friend to Transformations.
20 points - Attend a special Transformations event.*
30 points - Hitting your goal weight.
30 points - Maintaining your weight 6 months after goal.** 
 
*Transformations special events may include a monthly
Transformers meeting or an event with a guest speaker.

**The 30 maintenance points will be rewarded  
at 6 months and every 6 months following.

First 50 points - TRANSFORMATIONS GOODIE BAG  

(which will have some great Transformations items inside) 

 

Next 50 points (total 100 points) - FREE BOX OF FOOD

 

Next 50 points (total 150 points) - MAINTENANCE COOK BOOK & A TRANSFORMATIONS T-SHIRT

 

Next 50 points (total 200 points) - 2 BOXES OF FOOD

 

And there will be a special announcement later in the year where we will award a GRAND PRIZE for whoever has the highest point value at that time.
