May/2013

Skin cancer is one of the most common types of cancers, with more than 1 million people diagnosed each year. One in five Americans will develop skin cancer in their lifetime.

But there's good news... Skin cancer is one of the most preventative types of cancer. This month,
we are dedicating our newsletter to increasing awareness, early detection and prevention.

Be Sun Smart... Stay Protected!
Case of the Month:

Breast revisions are a commonly performed procedure. Patients present for a multitude of reasons to improve the shape or feel of their breasts. In many cases, the patient has had the breast implants for many years and due to changes in their body size, age or pregnancy the breast implants need to be replaced.

The most common reasons women with existing breast implants present for a revision are hardening of the breast implants due to capsular contraction, feeling of tightening or pulling, distorted look of the breast, rupture or deflation of the implant and rippling or wrinkling or change in the overall shape of the breast. This particular case, my patient experienced capsular contraction which is when scar tissue forms around the implant as an over active response to the implant. The scar tissue tightens and makes the breast round, firm and possibly painful.

The issue of capsular contracture can be treated in several ways. Typically if patients present for the first time with this scar they can be treated by removing all of the scar tissue and replacing the implant. However in patients that have had several episodes of scar tissue newer treatments have been developed to help prevent the scar tissue from forming again.

The first alternative is to remove the scar tissue completely and replace the implant, however in addition a material called strattice is placed around the implant that decreases the risk that the scar tissue will form around the implant. The new material prevents the scar tissue from being able to surround the implant and thus decreasing the risk of capsular contracture keeping the breast soft and natural. That is what this patient was treated with at the time of surgery.

The second alternative is to remove the breast implant completely and augment the breast with natural fat.  The patients undergoes removal of the implant and liposuction (from areas where they want fat removed, abdomen, flanks and so forth) then the fat is processed and injected into the breast to create an augmented breast using fat only and no breast implant.

The ABCD's of Skin Cancer

In a sense, you are your own dermatologist. A sudden or continuous change in the appearance of a mole is a sign that you should get it checked out. The ABCD Rule can help you remember the symptoms of melanoma:

Asymmetry
Is it asymmetrical?

Border
 Is the border irregular?

Color
Is there any color variation?

Diameter
Is it greater than six millimeters in diameter?

If you answered "yes" to any of these questions, you need to see a dermatologist. Although most of these suspicious moles may turn out to be normal and non-cancerous, it is better to be safe than to ignore it.
Q & A with Dr. Wolf

Q. I am a twenty year old girl and would like to ask you a question.  My mother keeps hassling me about lying out in the sun.  I have seen pictures of her when she was younger, and she spent a lot of time in the sun. She tells me that I am going to get wrinkled and get skin cancer.  My question is, is she just trying to scare me about getting skin cancer and wrinkles?  She is 53 years old and she looks great. What do you think?

A. Your mother is 100% right!Few people survive exposure to the sun without some sun damage.  If you want speed up the aging process, then bake yourself in the sun.

There has been a lot of research about sun exposure since your Mom greased up, put on her reflector, listened to music and chilled out on the beach.  It shows that the sun damages your skin  by destroying your elastic fibers as well as collagen fibers.  Your skin will get pigmented, dehydrated, thin out and just plain look bad!

Worse than that, there is a trend that shows a higher rate of cancer among younger people.  The two most common forms of cancer are basal cell and melanoma.  Basal cell cancers tend to grow slower and are easier to remove.  Melanoma can grow quickly and can metastasize (spread all over) if not caught early.  Young people DIE from this kind of cancer. The initial treatment for both cancers is to surgically remove them.  The surgery can leave scars if the cancerous area is big.

Here is the good news!  It can be prevented. Wear sunscreen! Don't spend your days laying out by the pool. If you need a little extra color, there's spray tans (which are perfectly safe!). Avoid sun beds!

Should you suspect that you have a skin cancer you should see a doctor that can look at a lesion and or biopsy (get a piece of it) it and send it to be analyzed.  Look for areas on your skin or moles that have changed color, changed shape, bleed, or just plain changed.  Sun yourself responsibly!

Treating Sun-Damaged Skin

Excessive exposure to the sun early in life can make a person look older than he/she really is (We should all know this by now!).

This premature wrinkling and skin damage from sun exposure is called photoaging.

Although there is no such thing as complete damage reversal, there are treatments and products available.

Chemical peels, topical medications such as tretinoin, and microdermabrasion and laser therapy are a few procedures and products used to treat sun-damaged skin.

To schedule a skin consultation with one of our medically trained aestheticians, Ana or Kelley, call us at 305.595.2969.
