The Western DuPage Special Recreation Association (WDSRA) is conducting a Needs Assessment Survey in order to better serve our families and communities. We want to know how we are currently meeting your needs and how we can improve on delivering outstanding service to you.

By taking this short survey, you can help us identify areas in which we can improve our programs and services, staff and customer service. It will take approximately 5-10 minutes to compete and your responses are confidential. We ask that you complete this survey by 4/17/15.
