At our recent Winter Symposium, more than 200 PeaceHealth leaders gathered to discuss our new care model, Care Together.
This model incorporates the theme we have been discussing within PHMG for some time now. That is, working the value equation in health care --
Value = Quality + Experience
Cost
Care Together talks about all of us contributing to the care of the individual patient every time, every touch.
It is manifested in three dimensions:
- Care Excellence (Quality)
- Care Experience (inclusive of Patients, Providers, and Caregivers)
- Care Performance (how we operate as an organization).
Each of us in PHMG contributes to producing a high-value experience for our patients and we are committed to doing so in the most efficient manner possible.
Underlying all of this is our new electronic medical record -- CareConnect, which enables our work together as a system.
