PHMG's Executive Team recently reaffirmed our commitment to our Patient-Centered Medical Home strategy, and specifically, to funding and nurturing our pilot sites during Fiscal Year 2015.
This commitment will support our re-envisioned team-based care model. Our plan is to incorporate behavioral health, preventative health and disease management into the development of team-based care at PHMG.
As we move forward, we're working with People & Culture to align recruitment strategy with our care transformation initiatives. We'll also support Provider Recruitment with onboarding and training our caregivers.
This team-based care model -- currently being tested in six PHMG pilot sites -- centers care on the patient with the goal to deliver quality care quickly and efficiently, while improving provider and caregiver satisfaction. Through the work of the pilot clinics, we will learn and improve our model and approach so that we can look to expand team-based care and population health strategies across the networks in the coming years.
