PeaceHealth Ketchikan Medical Center received the Centers for Medicare & Medicaid Services (CMS) Innovation Grant in 2012. More than three million dollars were awarded over three years to fund projects that improve access to care and health outcomes in target patient populations.

Ketchikan's primary focus with this grant is care coordination: connecting with patients upon hospital discharge and those with chronic diseases. Coordinators monitor patient progress, answer questions and link them to PCPs and social services within the community.

In the nine months that care coordination was fully implemented, readmission rates for PHMG patients were 27 percent lower compared with patients not part of the program. Ketchikan has also noticed a decrease in unnecessary ER visits, an increase in access for same-day appointments and a 30 percent increase in immunizations.

Through these collective efforts and preventative measures, reductions in unnecessary and costly treatments have resulted in a cost saving of $1.5 million over the past year. This is good news for our patients and health care system.
