Year One Update
Ketchikan Grant Results
PeaceHealth recently completed the first of a three-year project to improve healthcare in Southeast Alaska, funded by the $3.1 million grant from Centers for Medicare and Medicaid. Results so far?
  • 94% decrease in need to refer to emergency room due to lack of clinic availability
  • 28% improvement in immunization rate for children at age 2
  • 29% increase in patients with diabetes having a visit and being assigned a PCP within the last year  

The 12-month projection also shows significant cost savings to CMS and commercial payers.

This week I wanted to take a moment to recognize the relatively quiet, yet steady progress on addressing the results of last winter's provider engagement survey.

From the start, we wanted to act quickly on the high-impact themes while acknowledging there would be limitations in resources and capacity during a year of rolling out the new electronic health record.

Physician leaders and managers in every community have done a great job of working with local resources (especially Organizational Development staff and partners from Human Resources) to move forward on survey result action planning as quickly as priorities and circumstances allow. A few system-wide actions are already under way (see item below on changes to our Patient Experience measure).

Action is being planned in the following areas:
  • Practice operations (schedule flexibility, EMRs, clinical space, rooming efficiencies,billing)
  • Staffing (skills & training, recruitment, retention)
  • Compensation & Development (patient satisfaction, compensation)
  • Practice Management (provider opportunities, leadership enhancement)
  • Administration (system, community) 
  • Support for Practice (recognition)
  • Patient Care
View number of action items by theme. Because many steps are locally focused, please talk with your area leaders for more details.

Oregon West Network is past the bolus of work around Epic so leaders are in a better position to begin sharing their results and identifying actions.

While not distracted (yet) by Epic, Columbia Network has had its own unique challenges. Still, they hit the survey work hard and have a solid plan.

Northwest Network also hit the ground running. Implementation of next steps is planned and, in some cases, started. They will pause briefly for their Epic go-live next month.

While there's more to do, it is a good time to acknowledge what has been done so far. Please join me in thanking your communities' key players in this work and, if possible, offer your assistance. It makes a difference.
 

Patient Experience
New Measure Starting FY14
Next time you see the VISTA dashboard, you will notice that it reflects a different measure for Patient Experience. "Rate provider" is being replaced with "Provider Communication" composite starting in Fiscal Year 14.

Providers and managers can expect to hear more from their local PHMG leadership about this change and how the transition will be made from the old to the new measure. An audio presentation describing the change is also available on the PHMG Patient Experience site.

Diabetes Education
New Online Course Available

As medicine moves from a volume to value model our quality scores will become more transparent and our reimbursement will be dependent upon outcomes - both for organizations and individuals.  To perform at our best, we need to follow evidence-based algorithms for the care of our patients.

 

To help providers manage patients with diabetes, PHMG has developed a brief Computer-Based Training (CBT) module for our 2013 Diabetes Management Guidelines in Non-Pregnant Adults clinical algorithm to help providers keep up-to-date on the latest guidelines from the American Diabetes Association, including A1c, BP, and LDL recommendations that comprise our VISTA Diabetes Quality Bundle measure.

 

PeaceHealth St. John Medical Center has designated this CBT for .5 AMA PRA Category 1 credits for physicians; however, we encourage all providers and interested staff members to complete this educational opportunity. Also, please use the feedback form at the end of the CBT to send suggestions about how to improve diabetes care and additional tools you would find useful. You can find the Diabetes CBT under Resources on the PHMG Quality and Safety site. Our next CBT will address chronic pain and narcotic medication management.
