This Week's Q&A

Question:

After Epic is live in the clinics, how will hospitals and clinics share important patient care information documented in two separate EHR systems?

Answer:

Sharing of information will  be accomplished several ways.  Major elements of the record (like labs, imaging and visit notes) will exist both in Epic and the hospital systems (CE or McKesson). In addition, the hospital systems will still be available to those who work in the clinics, and we will provide access to Epic for those who work only in the hospitals. We are currently developing quick-access guides to Epic for those who need just read-only access to look up information. We are also working on other tools to make it easy to get to the patient's information in Epic.   Finally, work is being done to create Continuity of Care Documents (CCDs) that can be exchanged back and forth between users of Epic and those using other systems.  These CCDs will contain things like medication lists, allergies, problem lists and other important clinical information.  Read the high-level fact sheet (created for the Oregon network) on how information will be shared between hospital/

hospice/home care and clinics.

PeaceHealth Physicians & Providers,

Dr. Howard Graman
This past week, I've had the privilege of spending time with physicians and leaders from around the country at the annual American Medical Group Association (AMGA) conference. I look forward to sharing a few key takeaways and insights in the near future.

One area in health care that's getting a lot of attention is the trend toward transparency (see Dr. Sharpe's notes below). We'll continue to see more on this in the coming months.

Also below, you'll see a few thank you's have started coming in from the Doctors Day mailing. It's good to see. I hope you all hear from a few of your patients in the coming weeks.

Finally, I want to welcome Cottage Grove's  Dr. Lyle Torguson to his new role as Chief of Medical Affairs for the PeaceHealth hospital and medical group in that community.  And thank you to the previous chief, semi-retiring Dr. Jim Harrison, for his stalwart (and witty) service.

Sharing the Data
Transparency--the Most Important Driver in Care?
by Karen Sharpe, MD
Medical Director of Primary Care
Dr. Karen Sharpe Public reporting of medical group quality is rapidly gaining momentum and will soon become mandatory. The most recent evidence of this  is the ConsumerReportsHealth® ratings for 19 medical groups across Wisconsin on preventive services and vascular disease. The data was provided by the Wisconsin Collaborative for Healthcare Quality (WCHQ)--the benchmarking collaborative that PHMG uses for our diabetes triple bundle measure.

The 15-page report is well worth scanning. It is a good example of what patients will expect in the future as they make choices about where to get care--definitely a glimpse of things to come.

At PeaceHealth, Dr. Eleen  Kirman and I are working to help our group prepare for public reporting by 2014 and beyond. Implementation of Epic, our new electronic health record, and other initiatives will be a key part of moving us forward in this effort. You can expect to see ongoing updates on this work in the future.

While public reporting will be mandatory for (and meaningful to) the public, we're also keenly aware of its importance in recruitment, which is why we aim to leverage our efforts in this area to showcase PHMG's exceptional quality and service to attract the best physicians and providers to our group.

We look forward to partnering with you in our group's pursuit of high-quality health care. If you have a particular interest in transparency or data reporting, please shoot me an email.

"...if it weren't for (Dr. Warren Taranow's) dedication and expertise, it wouldn't have been much longer before I would have had my foot amputated..."
