Question: 

Will records be converted to Epic before go-live?

   

Answer:

Some elements of the medical records in our current system(s) will need to be manually converted to Epic at each community's go-live while other aspects of the records will be electronically converted to Epic. For more specifics, read the Guide to Interfaces and Conversions.

Today's PHMG Best Practices spotlights providers in Vancouver who have been working together for a while on innovative methods to improve diabetes care with great success. Their 6-month scores for the diabetes bundled measure range from 53.0% to 22.0%.

Diabetes Management: Using a Team Approach
This is the third installment in our three-part series on PHMG Best Practices on diabetes management that began in January.

This month we are featuring Diabetes Care from Dr. Robin Virgin in the Columbia Network.

All 17 Vancouver-based providers from two Vancouver clinics (Battle Ground and Fishers' Landing) have achieved a 6-month average on their VISTA DM Quality Bundle scores of 20.0% or higher, our target for FY13.

Congratulations to our colleagues in Vancouver for their exemplary success in diabetes management.
 
Epic Update
Siuslaw Prepares to Pilot Epic Ambulatory
Starting next week, the  "super users" from our pilot site will start learning Epic. "End-users" from that community will begin education on the system and workflows later this month. Check the Epic Education site for details.

The Epic team is making the necessary preparations to support go-live with our pilot site in Florence, OR on May 1. The February issue of Epic News outlines go-live activities and what everyone can expect when it is their turn to go live.

Work on "What Breaks" (an effort to identify the inter-regional and cross-facility processes and information exchange) also continues. The team wants to make sure providers and other caregivers who work outside of the clinics know what to expect after their local Epic go-live. Expect to hear more specifics by the end of this month.
