We're beginning a new monthly feature called PHMG Best Practices which will highlight examples of the some of the best work being done in our medical group. Our first comes from Dr. Ralph Fillingame, whose team has had exemplary success in diabetes management. We want to foster peer-to-peer support and help promote opportunities for everyone to learn from each other. It's one of the great advantages of working in a larger medical group. Diabetes: Getting Your Patients to Goal

As you know, the VISTA FY13 target for the Diabetes Quality Bundle is 20.0% of patients meeting all three measures for BP, A1C, and LDL  . From May to October 2012, on average, about 33.8% of Dr. Ralph Fillingame's patients have achieved all three individual targets. He shared a few secrets to the team's success. PQRS - We Need Your Signature!

PeaceHealth is once again preparing to submit data on three diabetes measures to the Centers for Medicare and Medicaid Services (CMS) for the Physician Quality Reporting System (PQRS). As part of the submission requirements, we need attestation signatures from about 250 PHMG providers for whom we are submitting data.

If you're one of those providers, next week you will receive a secure e-mail from Karen Sharpe, MD asking you to "sign" by clicking a link on the bottom of the page. Submitting this data makes PHMG eligible to receive an incentive payment from CMS. CMS is moving toward physician reimbursement that rewards value rather than volume. In addition to penalties for not reporting quality data, CMS will reward or penalize physicians and organizations based on relative cost and performance on over 100 quality measures beginning in 2015. New Care-Planning Model Going Live Jan. 22

PeaceHealth hospitals are implementing a new plan-of-care model on January 22, 2013. Using five universal goals to truly focus on the patient, the new model will improve experience and outcomes. The new plan-of-care model is part of the Care Transformation portfolio and provides the foundation for upcoming Care Transformation work, including implementing computerized provider order entry (CPOE) in PeaceHealth hospitals on Epic by June 30, 2017. The patient-care process redesign work that resulted in the new model is sponsored by the Patient Care Executives and led by the System Nurse Practice Council.
