STEP 1: Submit a Letter of Intent (LOI) by 5:00pm on November 30, 2012. To access the LOI form, instructions, and on-line technical assistance click here.

STEP 2: All applicants submitting a LOI in the required format and providing the requested information will be invited to submit a Project Summary and Budget Request, due on January 14, 2013. Information on these documents will become available on-line November 19, 2012.

STEP 3: Selected applicants will be invited to submit a Full Proposal, due on February 25, 2013.

HSCI funding priority will be given to community coalitions coordinating efforts to reduce racial/ethnic health disparities and to increase health equity in relation to Healthy Eating/Active Living (HE/AL) and Tobacco-Free strategies in rural areas of the state.

Eligibility Requirements:

South Carolina based (no out-of-state entities will be eligible)
Coalitions
Groups
Service organizations
Strategies address Healthy Eating/Active Living and/or Tobacco-Free Living

Examples of eligible organizations include: Non-profits, youth groups, schools/universities, civic associations, service clubs, health care organizations, faith-based organizations, parent groups, neighborhood associations, and local community coalitions.
