Please take a few minutes and complete the consumer survey available now.  It must be submitted no later than September 18, 2014.  Who should complete the survey?

  1. Nevada residents with Autism. Complete one for yourselves (Parents, you can assist your son or daughter if they are unable to complete it themselves, but please fill it out as they would in their own voice.  You should fill out a survey for yourself separately.)
  2. Parents/Family of Nevada resident with Autism. Complete one for each parent or family member responsible for a Nevada resident with Autism.
  3. Caregiver of Nevada resident with Autism.
  4. Advocate for Nevada resident(s) with Autism.
