Crossroad Transforming Patient Care
Crossroad's older adult patients now have immediate access to the services they need thanks to our partnership with the Council on Aging of Southwestern Ohio (COA). COA has devoted a full time nurse care manager to serve only Crossroad patients. Most importantly, the care manager is located at Crossroad. "This is especially important for our older adult patients," says Dr. Dave Rahner, Crossroad's Medical Director. "During a patient's visit, they know whether they are eligible for services such as home delivered meals and home health care before they leave." The care manager also makes home visits and will bring patients to their appointments at Crossroad. "Our partnership with Council on Aging not only helps us to better serve our patients," says Dr. Rahner, "it is transforming how we provide care to our patients." [More . . .] |

Did You Miss the Climb?
Thank you to the more than 150 Crossroad supporters and patients who climbed the 200 steps up Prospect Hill to raise funds for Crossroad's health care services to patients with no insurance. Some sprinted up the steps. Others chose a more leisurely approach. But whatever the pace, participants agreed that the 17th Annual Climb for Crossroad was a great way to raise money for a great cause. [More . . .]
Donations Still Needed
It's not too late to take steps for our community's health and help Crossroad provide lifesaving health care to those in need. Here's how your contribution helps:
- $25 provides a cervical cancer screening for a woman without insurance
- $100 provides a medical visit for a patient without insurance
- $500 provides yearly care for a patient with diabetes

West Side Office Gets a New Home
In May, Crossroad Pediatrics West moved into a larger office space located in a medical office building next to Mercy Health Western Hills Hospital. The new office has the capacity for Crossroad to add additional services, including mental health care and adult primary care.
