 "A lot of our lifts have forks that drag or slide on the floor, now we're told that some are unsafe because of wear and have to be replaced. How can we get our drivers to keep them off the floors?"
Primary lift chains should be checked and adjusted to see that the chain tension is equal on the left and right side. Check to insure there is no chain twist. Due to a combination of tire and chain wear, it may not be possible to prevent forks from touching the floor. The operator always has responsibility for preventing that contact during use.
Check chain wear, replace as needed. Then look at the wear on your tires, just because they're still round and black doesn't mean they don't need to be replaced.

 "Your technician tied off our cab doors here explaining that you have a lockout/tagout procedure for that?"
You becha we do! We are responsible for protecting our workers and customers from hazardous energy. The unexpected start up or release of stored energy [a swinging cab door for example] can result in serious injury or death to workers. We have quite a number of specific lockout/tagout [LOTO] procedures, and additional safe practice guidelines. The example our tech cited you is defined here:
Specific Procedure Steps:
Step 1: An authorized employee will identify a truck cab exists and understand the risk present if a cab door should close while repairs are being completed, and know the method to prevent this if no OEM mechanism is present.
Step 2: The cab doors will be fully opened.
Step 3: With the doors open, a length of rope will be used to tie them in place, either to the opposing door, or to another fixed object available on the equipment.
Step 4: The equipment will be disabled by insuring the ignition or key switch or other mechanism is in the off position. If the unit is a keyless unit see lockout/tag out procedure for securing keyless units. A lockout/tag out tag will be placed on the steering wheel to identify the unit as one that has been purposely disabled.

We can all agree that driving a car while distracted is risky behavior. Dan Clark, from The Safety Brief, accurately states that "Forklift safety starts when the driver understands a forklift is not a car." While they're definitely not the same, similar rules apply to both.
